(CLEVELAND CLINIC) Obesity is a major health problem in the U.S. affecting more than 40% of adults.
It is also a major risk factor for heart disease, which is the leading cause of death in the country.
Cleveland Clinic researchers looked into whether weight loss surgery could reduce some of these health problems.
Bariatric surgery can lead to significant weight loss and most patients can keep the weight off for a long period of time, said Ali Aminian, MD, Director of the Bariatric and Metabolic Institute at Cleveland Clinic.
Dr. Aminian and his team studied more than 7,000 patients with obesity and type 2 diabetesOne group underwent weight-loss surgery, and the other group received the usual care.
The research team found patients who underwent surgery were 40% less likely to suffer a major cardiovascular event, like a heart attack or stroke.
The study also found surgical patients only had to lose 10% of their weight to reduce their risk for a major cardiovascular event.
In comparison, patients who received the usual care had to lose 20% of their weight to see the same results.
Dr. Aminian said it shows there are benefits to metabolic surgery, beyond weight loss.
These changes that can happen after these types of procedures, including change in the hormones, in the bile acids, in the microbiome, these may explain the improvement in the metabolic profile of these patients after surgery, said Dr. Aminian. It can exceed those expected from weight loss alone.Based on this research, Cleveland Clinics team developed a calculator to help patients decide if weight loss surgery is right for them.
It looks at a persons individual risk factors and shows their risk of developing major heart complications over the next 10 years.

Researchers at the Zlotowski Center for Neuroscience at the Ben-Gurion University (BGU) in Israel recently discovered what they describe as "a neural subnetwork of connected regions between the brain and gastric basal electric frequency that correlates with future weight loss based on connectivity patterns." In lay terms, this means that people who see and smell food in a way that triggers their brain more enthusiastically are the same people who consistently overeat and gain weight.
The study looked at 92 people during an 18-month lifestyle weight loss intervention, led by Prof. Iris Shai of BGU's Department of Epidemiology. All had a large waist circumference and abnormal level of blood lipids (the fatty substances found in the blood, including cholesterol and triglycerides). "It appears that visual information may be an important factor triggering eating," principal investigator Prof. Galia Avidan, from the BGU Departments of Brain and Cognitive Sciences and Psychology, said in a statement. "This is reasonable, given that vision is the primary sense in humans."
The findings, which were published the journal Neuroimage, led the researchers to conclude that "weight loss is not merely a matter of willpower, but is actually connected to much more basic visual and olfactory cues."

Of course, weight management is a huge concern for many Americans. According to the latest data from the Centers for Disease Control and Prevention (CDC), 36.5 percent of American adults are obese, and another 32.5 percent are overweight. Additionally, 49 percent of U.S. adults surveyed between 2013 and 2016 reported trying to lose weight at some point during the prior 12 months, according to the CDC's National Center for Health Statistics (NCHS). Women were more likely to be making an effort to slim down (56.4 percent of women versus 41.7 percent of men).

There's no shortage of weight-loss plans out there. But how do you know which one's right for you? A new study from the University of Pennsylvania's Perelman School of Medicine suggests it may come down to the personality traits that make youwell, you.
For the study, published October 14 in the scientific journal PLOS ONE, a team of researchers led by X. Shirley Chen, M.D., re-analyzed data from a 2019 study that sought to identify the best way to motivate overweight and/or obese adults to become more physically active. The 602 participants, all of whom committed to walking more and wearing a step-counter to chart their progress, were randomized into three groups defined by their style of behavioral intervention:
Competitive: participants' step counts were posted on a leaderboard.
Collaborative: participants were assigned teams, and their step counts were combined.
Supportive: participants sent step reports to a designated friend or family member.
After 24 weeks with the intervention and 12 without, the competitive group's step total was significantly higher than the others. But when Dr. Chen reviewed the data, she observed wide variations among individuals and theorized that this might be due to their different personality traits. (Of course, there's also the factor of having a diet that consists of choices like the 100 Unhealthiest Foods on the Planet.)
To test this theory, Dr. Chen and her team used data collected at the beginning of the 2019 study to sort participants into three personality types:
Extroverted and motivated
Less social and less active
Less motivated and at-risk
Re-evaluating the 2019 data through the lens of "personality," Dr. Chen found that competitive weight loss plans were actually not the best plan for extroverted and motivated participants. Their step counts increased at first, only to drop off once the leaderboards were gone. By contrast, all weight loss plans worked for those who were less social and less active, while none worked for the less active and at-risk group.
While this research is too preliminary to draw conclusions about the best wellness plans for, say, extroverts, the conclusion to draw, Dr. Chen tells Eat This, Not That!, is that "one size does not fit all. Wellness plans could be more effective if customized for individuals."

SASKATOON -- Brianna Bowyer, 25, is trying to raise $18,000 for skin-removal surgeries after the province denied coverage for her following her rapid weight loss.
Tired of living overweight, in a body she wasnt proud of, Brianna Bowyer said she woke up one morning and decided she was going to do everything in power to shed weight, and live a healthier lifestyle.
I went to my mom and I said I gotta lose the weight. I'm going to lose 140 pounds , Im going to do it this time, and I started that day and I never looked back, Bowyer said.
At 22-years-old Bowyer said she was nearing the 300-pound mark. Determined not to allow her weight to stand in her way any longer, Bowyer said she kept a strict routine working out four hours a day, eating purely whole foods and living as healthy as she could.
During her weight loss, Bowyer said she consulted with her doctor after she lost the first 50 pounds.
I already knew with losing weight exactly what was going to happen with my skin, it was something that I was concerned about from day one, Bowyer said.
She said her doctor told her if she lost the weight, and kept it off for two years, then she would be referred to a plastic surgeon to perform the skin removal and the procedures, abdominoplasty and mastopexy, would be covered under the provincial health plan.
So I waited and I lost my weight and I continued to wait for my letter and it finally came, she said.
On Oct. 17, Bowyer showed up to her appointment and quickly found out her skin-removal surgeries would not be covered, at all, by the provincial health plan.
He told me no coverage. No coverage, thats it. He said that was no longer something that Sask. health does, it is now the requirement that you have to have an infection that requires you to stay in the hospital, Bowyer said.
Your skin has to become infected, or bad enough that you are in the hospital and that its threatening your life, she added.
In an email, the province's health ministry said abdominoplasty is not listed under the Saskatchewan Physician Payment Schedule, therefore the ministry has no authority to make payment for the service. The ministry added cosmetic procedures are not insured under the provincial plan.
The ministry said a procedure that is covered under the provincial plan is abdominal panniculectomy, a surgical procedure to remove excess skin and tissue from the lower abdomen that is insured only when specific medical requirements, such as chronic and recurrent skin conditions, have been met.
Bowyer said she spends hours every week taking care of her excess skin to ensure no infections would appear.
I spend hours just showering, cleaning underneath my skin. Every little flap has to be cleaned, has to be dried, has to be lotioned, she said, adding shes battled two bacterial infections in her belly-button since the weight loss.
Bowyer learned the skin-removal surgeries would cost her $18,000, plus a six to eight-week recovery time. Her mother, Gloria Bowyer set up a GoFundMe Page to help her daughter pay for the surgeries she needs to return to a normal life.
The way it makes me feel is just awful. I wanted to know what it was like to be in a body I wanted and I dont, I was left with something that I never expected to be this bad, she said.

Going for a bike ride is good exercise, and it can be as easy or as difficult as you make it. Plus, if you can't get outside, you can even find many of the same health benefits on a stationary bike indoors.
Here are five major health benefits of cycling and how to bike safely.
The U.S. Department of Health and Human Services recommends that adults do 150 to 300 minutes per week of moderate-intensity aerobic activity, or 75 to 150 minutes of vigorous-intensity aerobic physical activity. Cycling is one way to get aerobic exercise also known as cardio which gets your heart and lungs working.
For example, a large 2017 study looked at the benefits of "active commuting" and found that cycling to work was associated with a lower risk of cardiovascular disease, cancer, and death. In fact, the health benefits of cycling to work may be even greater than the benefits of walking to work.
Moderate or vigorous aerobic activity like cycling can help you lose weight. A 2019 research review found that indoor cycling, when combined with healthy eating practices, was recommended to help people lose weight, reduce blood pressure, and improve lipid profile.
The amount of calories cycling burns depends on how hard you're working. For example, a 185-pound person can burn the following number of calories per hour:
Because cycling is a low-impact activity, it's easy on the joints even easier than walking.
"When we walk, and when our foot hits the ground, because of gravity, we hit with two to four times our body weight," says Curtis Cramblett, a licensed physical therapist and certified cycling coach, strength and conditioning coach, and bike fitting educator.
On a bike, there is still some force on the joints, depending on how hard you're pushing on the pedals, but cycling does not deliver the compressive force that walking does, Cramblett says. This makes it suitable for people who may have injuries, such as a knee or hip replacement, or a lower back problem.
"Because it's low impact and because you're sitting on a saddle, it can be as gentle as you like," Cramblett says.
A small 2013 study found that bicycling can improve balance and even help prevent falls for older adults. However, these benefits likely apply only to cycling outdoors, and not for indoor stationary cycling.
"When you're trying to keep a bike upright, you're working on balance," Cramblett says. That's why cycling is often recommended for people with neurological disorders to train balance and coordination. In fact, a small 2015 study found that stationary cycling can improve balance in stroke patients.
"Once you learn to ride a bike, you can always do it. It really brings back some of the coordination that we learned as a kid around balance," Cramblett says.
It is well-proven that exercise such as cycling can improve cognitive functioning, reduce depression, and enhance overall well-being.
Plus, when you ride a bike outdoors, it may enhance these benefits. A 2013 research review found that exercising in natural environments provides greater mental health benefits than indoor exercise does.
The most important benefit of cycling might be the same one you appreciated as a kid. "Think about the first couple of times you were on a bike: the freedom, the joy, the play, the smile, the laughter there's an emotion that comes into that old memory," Cramblett says.
For beginners, Cramblett recommends starting out slow maybe once or twice a week for 15 or 20 minutes.
"Then you can work your way up, giving your tissues, your tendons, your muscles, your joints, an opportunity to get used to the activity," he says. People often jump into activities too fast and do too much, but that can cause injury and make it a chore rather than something you enjoy, he says.
Comfort is also important. If the position of your saddle or handlebars is off, "it puts undue stress on your joints," Cramblett says. Getting your bike professionally fitted can prevent pain and injury, and this applies to both outdoor bikes and stationary bikes.

Whether you head outdoors or use a stationary bike at home, you should do what works for you. Sticking to an exercise regimen is much easier if it's fun, Cramblett says. "The most important thing is that it's something that puts a smile on your face and that you're willing to come back and do tomorrow."

"I like looking at weight loss as making long-lasting small lifestyle changes, versus doing sweeping changes that you might be less likely to stick to," Gorin says.
Whenever you are considering changing your diet, always consult your doctor and consider consulting with a dietitian or nutritionist before making a drastic change. "Don't jump on whatever diet plan your friends are doing. Speak with a nutrition professional who can help you identify the eating plan that is most suitable for you. Go with a program that is sustainable, even if the weight loss is slower," Dr. Peter says.
Whole30 is a variation on the elimination diet that cuts out grains, dairy, and other refined foods.
Whole30 is a popular challenge developed by sports nutritionist Melissa Hartwig-Urban. It's basically a form of the elimination diet, where you remove certain foods for a period of time that could be causing adverse medical symptoms, like a recurring upset stomach. Although it's not technically marketed or meant for weight loss, some people do lose weight during the 30-day challenge.
If you use a Whole30 challenge as a tool for identifying food sensitivities or foods that trigger negative symptoms, it could be helpful for you. "[Whole30] is a structured elimination diet similar to paleo and helpful for identifying food triggers," Dr. Peter says.
"The diet is so restrictive -- if you slip up even once and eat a bite of pizza or a spoonful of ice cream, you have to reset and start over on the diet. I haven't found this type of unforgiving mentality to work with my clients," Gorin says.
When it comes to weight loss, Whole30 is pretty restrictive and hard to follow, meaning the chances you'll be able to stick to it are pretty low. Also, since Whole30 isn't designed for weight loss, you can technically follow the plan and not lose any weight, if you aren't also paying attention to portion sizes and macronutrient ratios.
The Paleo diet is a style of eating that mimics what we think our cavemen ancestors followed -- aka a hunter-gatherer style of eating. The main tenets of the diet involve eating fruits, veggies and meats mainly, and nixing dairy, grains, beans, wheat and some other foods.
"[The Paleo diet] excludes processed foods, inflammatory foods such as grains, dairy, refined sugar, foods high in lectins. It can be very beneficial for reducing inflammation, especially in people who have autoimmune conditions," says Dr. Peter. Some studies show that the diet can be helpful for weight loss (although there needs to be more research).
The Paleo diet requires you to cut out grains, which is a controversial topic in the nutrition world. Some say that the benefits of grains outweigh the potential negative effects, and that removing all grains is too restrictive. According to Peter, removing grains from your diet, "can enhance mineral absorption. Grains contain phytates, which can interfere with mineral absorption."
"Some research suggests that the eating style could lead to short-term improvements in waist circumference and fasting blood sugar, risk factors for chronic diseases. But there hasn't been a lot of research on the diet, especially on its long-term benefits. For this reason, I don't think it's the best approach to weight loss, especially considering how restrictive it is," Gorin says.
Peter also points out that the Paleo diet can be tough to transition to, especially if you are used to the Standard American Diet.
The Keto Diet is a high-fat diet where the goal is to reach ketosis.
The Keto diet is one of the most popular (and controversial) diets today. It's also one of the more extreme diets since it involves eating a huge amount of fat in comparison to other nutrient groups, and it almost completely cuts out carbs (like fruit and sweet potatoes) from your diet. The goal of the keto diet is to restrict carbs enough that your body goes into ketosis, which is a state where your body begins to burn fat instead of sugar for energy.
"Certain people may find this diet helpful. For instance, people with Type 2 diabetes could find a benefit -- as some research shows that a higher protein and higher fat diet like the keto diet may help manage hunger levels. The diet could also help increase A1C levels," Gorin says.
"This is not a diet I'd recommend for most people. It is extremely difficult to follow, especially long-term. We also don't have a clear picture of long-term benefits of weight loss," Gorin says.
Because of the restrictive nature of the diet, Gorin says you're more prone to certain nutrient deficiencies, including fiber and sodium. One of the effects of the Keto diet is lower insulin levels, which plays an important role in how your body regulates sodium. Low-carb diets cause your body's insulin levels to go down, which causes your body to flush excess water and sodium.
That might sound harmless, but your body needs a proper balance of sodium to function. "In extreme cases, this [low sodium level] could lead to more severe side effects such as seizures, coma and death," Gorin says.
Other people that Gorin says the diet will not work for is those with a history of disordered eating, people with kidney disease or a history of experiencing kidney stones and people with Type 1 diabetes.
Whether followed for health or ethical reasons, a vegan diet is a popular diet that removes all animal products from the diet. This means the diet is made up of mainly fruits, vegetables, grains and beans and nixes meat, fish, eggs, dairy and any other food products that contain any ingredients that come from animals.
"People who follow this type of diet see benefits in weight control, as well as a reduced risk for Type 2 diabetes and coronary heart disease," Gorin says.
"A balanced vegan diet can be challenging to follow, so make sure that you're eating a variety of nutrients and getting ample protein. There are certain nutrients that are more difficult to get from a vegan diet, such as vitamin B12 and omega-3s, so you may want to consider taking certain supplements," Gorin says.
Peter adds that one challenge with veganism is getting enough amino acids. "Amino acids are the building blocks of proteins. Essential amino acids can only come from food and cannot be made by the body. Methionine is a common amino acid deficiency in vegans that is needed for brain and liver function," Dr. Peter says.
She also cautions that some vegans can end up eating more carbs than is beneficial, which can cause blood sugar and weight control issues for some people. "[Vegans] can also develop other nutritional deficiencies like choline, zinc, Iron, B12, vitamin D and chromium," Dr. Peter says.
The Mediterranean Diet includes lots of fruits, veggies and fish.
The Mediterranean diet is a style of eating that is based on the dietary habits of countries that border the Meditteranean Sea -- most prominently Greece and Italy. At the time the diet was created, those countries experienced significantly lower levels of heart disease compared to other countries, like the United States. The diet emphasizes foods like fish, vegetables, fruit, nuts, beans and whole grains, with some smaller amounts of red meat and dairy.
"This is really more of a lifestyle than a diet, and I'd recommend it to pretty much anyone. There is much research to show that the Mediterranean diet can be helpful for health as well as weight loss," Gorin says.
Neither dietician cited any concerns over the diet since it is well-backed by research and is a favorite of many experts. One reason is because it's more of a balanced lifestyle, instead of a restrictive diet that limits large food groups.

According to health experts, obesity is a major health problem in the U.S. affecting more than 40% of adults.
Cleveland Clinic researchers looked into whether weight loss surgery could reduce some of these health problems.
They studied more than 7,000 patients with obesity and type two diabetes.
One group underwent weight loss surgery and the other group received the usual care.
The research team found patients who underwent surgery were 40% less likely to suffer a major cardiovascular event like a heart attack or stroke.
The study also found surgical patients only had to lose 10% of their weight to reduce their risk for a major cardio-vascular event.
These changes that can happen after these types of producers, including change in the hormones, in the bile acides, in the micro biome, these may explain the improvement in metabolic profile of these patients after surgery. It can exceed those expected from weight loss alone, said Dr. Ali Aminian, Director of Bariatric & Metabolic Institute at the Cleveland Clinic.
Based on this research, Cleveland Clinics team developed a calculator to help patients decide if weight loss surgery is right for them.
