Dear Amy: My husband had weight loss surgery about five years ago and, although hes lost an incredible amount of weight, no one prepared me for the extreme psychological changes.
He is healthier and has more energy and confidence, but the negative changes blew me away.
He has become obsessive-compulsive. He writes down everything that goes into his mouth. He weighs himself every morning completely naked and documents it to the ounce. He has become self-absorbed, and is worrying only about himself.
Along with that, he is going through a midlife crisis. He bought a couple of sports cars and cruises around on the weekends. He goes out a couple times a week by himself for a few drinks.
He has such a high opinion of himself, he could be cheating for all I know, since our sex life has changed; I cant get used to how he looks.
Hes lost so much weight that he looks like an old man. His skin is hanging off of his body and he will not have it removed.
Everyone tells him he looks good to his face, but they tell ME hes way too thin, or they ask if hes sick.
Ive checked some weight-loss websites, and Im reading about the effects of extreme weight loss on a marriage and family.
Apparently, Im not alone. We tried counseling, but he refuses to admit the change in him. He blames ME for not accepting him since the weight loss.
I am now self-counseling, reading others stories, and trying to learn how to cope.
What do you think, Amy?

Dear Distressed: The psychological impact of extreme weight loss is being increasingly studied, because our current obesity epidemic is making extreme obesity, and extreme weight loss, more common.
Some of your husbands habits (keeping a detailed food and weight diary, for instance), are recommended after surgery as a way to keep the weight off. His other habit drinking alcohol is NOT recommended. And going out a couple times a week without you is not good for your relationship. Plastic surgery to remove extra skin is very expensive, and carries some risk.
He may have slipped into compulsive behavior or an eating disorder, but you do need to understand that this change has brought on a whole-life transformation (for him), that is altering not only his own physique, health, and outlook, but also the way the world relates to him.
There is no question that some of your husbands behavior is not good for your marriage, and yet you are completely focused on him and his changes, without understanding that in order to stay together, you will also need to change.
You may be mourning the man your husband was before his weight loss, but that man is gone. The guy who replaced him might be a jerk, but if you want to stay together you should both focus on change and compromise.
Dear Amy: What do you think of people who have long coughing fits in restaurants or coffee shops where others are eating?
I say that they should get up and excuse themselves until theyre done, or, if the cough is chronic, they shouldnt come out at all until theyre well. They should do this out of consideration for other diners, who might catch whatever they have.
My wife thinks I lack compassion, although she agrees they should at least excuse themselves.

Dear Unempathetic: I agree with your wife. Unless it is your waiter who suffers from a coughing fit while serving you, your first reaction should be one of compassion, rather than assuming that you might catch whatever illness the person has.
And if you have a suppressed immune system, making you susceptible to illness, maybe it is you who should stay home.
The thing about a coughing fit is that it is a fit. It comes on suddenly, and the person coughing assumes that it will end soon. Be nicer!

After high school, Ronnie Brower felt depressed. He wondered what he was doing with his life and turned to drugs, alcohol and food to soothe his emotions. As time passed, he slowly put on weight and eventually reached 675 pounds by 2013. When he went to the doctor, he learned something shocking: If he didnt lose weight, he might die.
That really hit me, the bus driver in Syracuse, New York, told TODAY.
But he ignored it for several days until a friend recommended that Brower talk to her cousin, Nick Murphy, who was a trainer. After stalling a little longer, Brower reached out.
I was a little nervous meeting him, he is huge. His arms are bigger than my whole body, Brower, 32, said.
After a doctor told Ronnie Brower, who was 675 pounds, that if he didn't lose weight he could die, Brower finally started working out and eating healthy foods. He dropped 458 pounds in 23 months.
But Murphy encouraged Brower and came up with a weight-loss plan. At first, Brower could only work out at home. Hed simply stand up and sit down in a chair, use an arm bike, do some arm raises with weights and attempted sit-ups.
After 100 days, Brower felt comfortable enough for the gym. He also started a ketogenic diet, a low-carb, high-fat, moderate protein and vegetable-focused diet. He hoped to get to 250 pounds.
When Brower first started losing weight he wasnt sure how much he lost because he was too heavy for his scale. He went to his uncles trucking company where he learned he lost 30 pounds in the first week. But that was mostly water weight and he soon learned dropping pounds from his 6-foot frame wasnt always easy.
I would get a little discouraged, he said. Some weeks I wouldnt lose any weight. Some weeks I lost 20 pounds.
He started sharing his weight-loss journey on a Facebook page and in early 2015, he received a message from a woman named Andrea. She, too, was losing weight and working with Murphys wife, Jill.
Andrea said she was proud of me and good job, Brower said.
When Andrea Brower went on a vacation she felt uncomfortable in her bathing suit. That's when she knew she had to lose weight, eventually dropping 120 pounds.
He responded with an aloof, thanks, you, too.
It was kind of lame, he said, laughing.
But Andrea, didnt let that stop her. She messaged him again and this time he actually chatted with her.
At the time, Andrea sent Brower a message, she was about eight months into her own weight-loss journey and had shed about 75 pounds. When she first started working with Jill, she weighed 250 pounds at 5 feet 4 inches. She hoped to get to 130 pounds. But like Brower, she struggled at first.
It was really difficult because I was so heavy, she said. "I was embarrassed that I am a big girl and going into the gym.
While Andrea was a thin child, when she was in eighth grade she started gaining weight, but like Brower, she really gained after high school.
I ate just awful. Just crap food and so I would also eat if anything would happen in life. If something great happened I would eat. If something bad happened I would eat, said the 25-year-old hairdresser.
Andrea started following Ronnie's weight loss online and started chatting with him. They had so many things in common, including shedding hundreds of pounds to transform their health.
After going on vacation and feeling very uncomfortable in her bathing suit, Andrea decided to lose weight. She had a business card for her trainer, Jill, for six months before she finally felt courageous enough to call in 2014.
Thats when she started working out and following a ketogenic diet, too. When she first spotted Brower in the gym she was immediately interested.
We just kind of started talking. Honestly, from there it was over with. I dont want to sound clich but it was love in first sight, she said. We had so much in common.
As the relationship blossomed, the two continued toward their weight-loss goals. Andrea lost 120 pounds and Brower shed 458, a total loss of 578 pounds combined. The couple has maintained it for year and a half and still work out and cook healthy meals together.
On May 13, the couple took their relationship to the next level and got married.
The wedding was phenomenal. I cried when she came down the aisle, said Brower.
After bonding over their mutual weight loss, the Browers realized that they had so much and common and they made each other happy. They married in May.
Andrea agrees.
It was amazing, she said.
They went to Mexico for their honeymoon and returned to Syracuse to continue their healthy lives together. They provided tips for others hoping to lose weight.
When it felt tough in the gym or the number on the scale didnt move, Andrea focused on a mantra that kept her motivated.
There were days toward the end where I was like Im done and I just remembered that I have a goal that I needed to reach, she said.
While the Browers had trainers keeping them focused, they also encouraged each other.
We hold each other accountable. And we know each other so well. Shes been through the same thing that I have been through, Brower said.
The couple lost weight by following a ketogenic diet and enjoy cooking healthy meals together.
When the couple became serious, they talked about their hopes and goals. And during this conversation, Brower stressed that working out would always be an important part of his life.
I need to work out to keep healthy for me, her, and our marriage, he said.
Not surprisingly, Andrea agreed.

Alcoholic drinks are normally the first thing that is advised to be cut out of any diet.
However, it seems one famous beverage could actually help with weight loss.
Tequila has been found to contain elements that could help anyone on a diet to lose weight and also fight diabetes, according to one study.
It's an exciting discovery for those who struggle to remove alcohol from their diet.
The research by The American Chemical Society explored the use of tequila and the agave plant that it comes from.
Agavins, natural sugars found in the agave plant, don't affect blood sugar levels due to being non-digestible.
This means they don't raise blood sugar levels like previously believed.
It also curbs hunger pangs so people feel fuller yet eat less compared to others sweeteners such as fructose and sucrose.

Agavins are not expensive and they have no known side effects, except for those few people who cannot tolerate them.

The non-digestible agavins could then be used to create new sugar variations.
Dr Mercedes G. Lpez said: We believe that agavins have a great potential as light sweeteners since they are sugars, highly soluble, have a low glycemic index, and a neutral taste, but most important, they are not metabolised by humans.
She goes on to describe how they can be used to help obese people and those with type 2 diabetes.
She said: "Agavins are not expensive and they have no known side effects, except for those few people who cannot tolerate them."

However, like with the drink, it should all be taken with a pinch of salt.
One personal trainer explains that a diet only works with energy balance so the energy coming in must be less than the energy going out to lose weight.

S.A. woman's 400-pound TV weight-loss transformation reportedly causing marriage problems
Over the course of three years, a San Antonio woman was able to lose more than 400 pounds, but she may now consequently lose her husband as her journey continues to unfold on national television and on social media.
The weight loss journey of 43-year-old Laura Perez has been documented in the TLC series "My 600lb Life." She weighed in at 594 pounds to start the show in March 2015.
When she met her husband, Joey, she was weighing more than 300 pounds, according to the Daily Mail.

The KHOU11 Verify team is tackling some of those home remedy rumors that spread like wildfire on social media. Have you seen the posts claiming cayenne pepper can help you lose weight?
It would be a dream come true for Texans but is it really true? Our Verify fact-checkers dig dome digging and there have actually been several studies on the matter.
Researchers in one of those studies found people who consumed red pepper with every meal felt more full and had fewer cravings, but that study only involved 15 people.
Another study found women who ate fresh chili pepper raised their metabolic rate for 30 minutes, helping burn more calories.
That was a double blind study with close to 80 people some that one was much more conclusive.
So our fact-checkers verify this one is for real, spicing up your diet will help you burn calories.

TOPEKA, Kan. (KSNT)- In a world of more weight loss programs than we can count, Laura Davis from Lyndon, Kansas, thanks her 71 pound weight loss to a unique program found in locally.
Two years ago this May, I started at TOPS and weighed in a 216. I felt the effects of being big. I would have knee pain several times during the week and this past may I weighed in at 145, said Laura Davis.
TOPS stands for, Take Off Pounds Sensibly, and has been around since 1948.
We first weigh in at the private weigh-ins. And then after the weigh-ins we go into our meetings. We talk about health, we talk about accountability, and we talk about support for each other, said Jerry Hardwick, a Northeast Kansas TOPS Chapter Leader.
But what really makes this program so enticing to users like Laura, is the no frills atmosphere.
Some of the other groups are just focused on their program, their meetings, their products and you dont get that. TOPS is more like a family, said Davis.
As a family, the group not only encourages good nutrition and exercise, but also a realistic routine. Two years later, Laura can now tell when shes off her weekly schedule.
I could tell if I hadnt had my water all day, I could tell if I hadnt worked out in two days, I could feel it.
Now, when Laura looks in the mirror, 71 pounds lighter, she says its kind of hard to believe thats her.
Oh yea! Thats me!
TOPS has an annual fee of $32 per year with meetings once a week.
