DEAR MAYO CLINIC: Every year, I make one or more New Year's resolutions that I have every intention of starting Jan. 1. But I find myself losing momentum quickly into the new year when the holiday merriment starts to fade. Do you have tips for achieving resolutions and goals?

ANSWER: It is the time of year again when people make New Year's resolutions, many of which are health related. Losing weight, eating healthier, exercising and quitting smoking are popular choices. Losing weight often leads the list, but it also is the most difficult to accomplish.

New Year's resolutions are easy to make, but not so easy to achieve. To make the resolution into an actual solution, you need to spend time preparing and planning. There is no magic button for keeping a resolution.

The first step toward success is developing a structured health plan. Your health plan should include where you're headed and why you want to get there. Be realistic about what challenges you may face along the way and define how you plan to work through the challenges.

The second step is to visualize your goals. Think about a what a healthy future might look like to you. Convert those thoughts into a short, clear statement: your health vision.

While there are many ways to write a health vision, one popular format is: "I want to (blank) so I (blank)." Examples may be: "I want to lose weight so I have more energy to enjoy life." Or, "I want to have better balance between my work and personal life so I have more quality time for myself and for my family."

After you create a health vision, the third step is to set SMART goals. SMART is an acronym that is all about achieving identified goals to produce a feeling of accomplishment and much-needed motivation to fuel your health journey.

SMART means:

Specific -- What am I going to do? You need to have a specific plan in place to start. Take the time to design and research.

Measurable -- How will I track my progress? You may say, "I would like to lose some weight." But it would be better if you said, "I want to lose 50 pounds in four months."

Achievable -- What steps will I take to make this happen? Set a realistic goal. With the right weight-loss program, 50 pounds may be realistic. Ask the program you're considering what the average weight loss among participants is after one year. Depending on the program and its weight loss approach, you may need to set a smaller, more attainable goal.

Relevant -- Is this important enough to me to want to do it? Think positively. Behavior only changes from the positive. Remember there are programs to help you accomplish your goal.

Time-framed -- When will I do this? Set a specific target date. There is nothing magical about Jan. 1 as a start date. It is better to pick a date when you have a plan in place.

Your SMART goals should consist of long-term and short-term goals. Establish your long-term goal first. An example of a long-term goal is: "I will walk briskly for 30 minutes each day at least five days a week."

However, if you haven't been exercising on a regular basis, there often is a need for intermediary, short-term goals. For example: "I will walk 15 minutes during my lunch hour at least three days each week for the next month." After reaching your short-term goal, extend your efforts gradually, until you ultimately accomplish your long-term goal.

Finally, accept challenges, and don't give up. Nearly everyone will face challenges during their health journey. Perhaps it's a busy family life, work, school, medical issues or peer pressure to continue bad habits. It's important to identify potential challenges and envision strategies to address them as part of an effective health plan.

Here are a few tips for success:

Confront your temptations. You will need to change your environment to help with your weight goals. Get rid of food that won't help you realize your goal -- this should be part of your planning.

Tell your friends. Changing your habits for good may affect your inner circle of friends. Let them know what you're trying to do and enlist their support in helping you achieve your goal.

Remember the reward. Make a list of the reasons you want to lose weight. When you lack reasons to change, it is easy to fall back into old habits.

Be prepared for a setback. Give yourself another chance. Most people slip up at some point. The people who are successful are the ones who get back on track. Look at how many days you still have left in the calendar year and see what you can accomplish before the year is up. Don't give up.

Today, the database has DLW data from over 7,600 people, rendering it the worlds largest collection.

Free and accessible to researchers with defined management-approved questions, the IAEA DLW Database contains information on diverse cases, ranging from athletes to cancer patients and people with cerebral palsy.

As most of the data comes from studies conducted in Western countries, such as the United States and the Netherlands, the IAEA is looking to expand the dataset further, to include Asia, Africa and Latin America.

Next year it will starta coordinated research project to add more lower-income country data.

Our database is an invaluable asset towards better understanding how the human body functions. This exercise study is a great example; whereas most DLW studies generally involve about 30 subjects, the exercise study had over 1,600, making the data very robust, said Ms. Alford.

The data in the IAEA DLW database is an untapped treasure trove, and we encourage researchers to reach out to us to gain access to its contents and contribute their own datasets.

It was then Morris began his journey back to racing, stripping the weight in a remarkable way.

We were sitting there, I didnt have a (riding) license at the time, and we were having some bets and I was complaining about one of the rides, Morris said.

(Johns) looked at me and said look at you sitting there saying that, you couldnt ride it any better.

I was pretty keen to get his cash so thats when I started to get back into it. I was doing a bit of boxing and a lot of walking.

I lost 18kg in a few weeks. All I ate was ice cream for three weeks. That is literally all I had, I only ate ice cream.

I was back riding not long after that, so I got his cash.

Jumping from barrier three, Morris says it is now in his hands before adding that he would be just as happy to see either Mishani Warfare ($101) or Mishani Persuasion ($126) win for Crooks.

I spoke to Simone Schultz from the stables before the barrier draw and I said I wanted something between one to five, she drew three so she rang me back straight after it and said I drew right in the middle for you so dont f*** it up, Morris laughed.

We are a team at Mishani , we have a good team at the farm, we have a good team at Eagle Farm and everywhere else and believe me everyone there plays a massive role.

It will be boom or bust for Morris on Magic Millions day as Mishani Spartan is his only ride on the nine-race card.

Testosterone replacement therapy is used for the treatment of age-related male hypogonadism, and prostate-specific antigen (PSA) is a primary screening tool for prostate cancer. The systematic review and meta-analysis aimed to determine the effect of testosterone replacement therapy on PSA levels.Medline, Cochrane Library, EMBASE, and Google Scholar databases were searched until February 28, 2014, and inclusion criteria were as follows: randomized controlled trial; intervention group received testosterone/androgen replacement therapy; control group did not receive treatment; and no history of prostate cancer. The primary outcome was change of PSA level between before and after treatment. Secondary outcomes were elevated PSA level after treatment, and the number of patients who developed prostate cancer.After initially identifying 511 articles, 15 studies with a total of 739 patients that received testosterone replacement and 385 controls were included. The duration of treatment ranged from 3 to 12 months. Patients treated with testosterone tended to have higher PSA levels, and thus a greater change than those that received control treatments (difference in means of PSA levels = 0.154, 95% confidence interval [CI] 0.069 to 0.238, P < 0.001). The difference in means of PSA levels were significant higher for patients that received testosterone intramuscularly (IM) than controls (difference in means of PSA levels = 0.271, 95% CI 0.117-0.425, P = 0.001). Elevated PSA levels after treatment were similar between patients that received treatment and controls (odds ratio [OR] = 1.02, 95% CI 0.48-2.20, P = 0.953). Only 3 studies provided data with respect to the development of prostate cancer, and rates were similar between those that received treatment and controls.Testosterone replacement therapy does not increase PSA levels in men being treated for hypogonadism, except when it is given IM and even the increase with IM administration is minimal.

Transgender hormone therapy, also called Hormone replacement therapy (HRT) or gender-affirming hormone therapy (GAHT), is a form of hormone therapy in which sex hormones and other hormonal medications are administered to transgender or gender nonconforming individuals for the purpose of more closely aligning their secondary sexual characteristics with their gender identity. This form of hormone therapy is given as one of two types, based on whether the goal of treatment is feminization or masculinization:

Some intersex people may also undergo hormone therapy, either starting in childhood to confirm the sex they were assigned at birth, or later in order to align their sex with their gender identity. Non-binary people may also engage in hormone therapy in order to achieve a desired balance of sex hormones or to pass as a desired gender. [1]

The formal requirements to begin gender-affirming hormone therapy vary widely depending on geographic location and specific institution. Gender affirming hormones can be prescribed by a wide range of medical providers including, but not limited to, primary care physicians, endocrinologists, and obstetrician-gynecologists.[2]

Historically, many health centers required a psychiatric evaluation and/or a letter from a therapist before beginning therapy. Many centers now use an informed consent model that does not require any routine formal psychiatric evaluation but instead focuses on reducing barriers to care by ensuring a person can understand the risks, benefits, alternatives, unknowns, limitations, and risks of no treatment.[3] Some LGBT health organizations (notably Chicago's Howard Brown Health Center[4] and Planned Parenthood[5]) advocate for this type of informed consent model.

The Standards of Care for the Health of Transsexual, Transgender, and Gender Nonconforming People (SOC) require that patients seeking gender-affirmation hormone therapy be evaluated for gender dysphoria by either a mental health professional or hormone provider who is qualified in the area of transgender care. The Standards also require that the patient give informed consent, in other words, that they consent to the treatment after being fully informed of the risks involved.[6] Before beginning gender-affirming hormone therapy, the patient must be evaluated for significant medical and mental health concerns. If present, these must be addressed and reasonably well-controlled.[6]

The World Professional Association for Transgender Health (WPATH) Standards of Care, 7th edition, note that both of these approaches to care are appropriate.[2]

Many international guidelines and institutions require persistent, well-documented gender dysphoria as a pre-requisite to starting gender-affirmation therapy. Gender dysphoria refers to the psychological discomfort or distress that an individual can experience if their sex assigned at birth is incongruent with that person's gender identity.[6] Signs of gender dysphoria can include comorbid mental health stressors such as depression, anxiety, low self-esteem, and social isolation.[7] It is important to note that not all gender nonconforming individuals experience gender dysphoria.

The World Professional Association for Transgender Health (WPATH) and the Endocrine Society formulated guidelines that created a foundation for health care providers to care for transgender patients.[8] UCSF guidelines are also used.[9][citation needed] There is no generally agreed-upon set of guidelines, however.[citation needed]

Adolescents experiencing gender dysphoria may opt to undergo puberty-suppressing hormone therapy at the onset of puberty. The Standards of Care set forth by WPATH recommend individuals pursuing puberty-suppressing hormone therapy wait until at least experiencing Tanner Stage 2 pubertal development.[6] Tanner Stage 2 is defined by the appearance of scant pubic hair, breast bud development, and/or slight testicular growth.[10] WPATH classifies puberty-suppressing hormone therapy as a "fully reversible" intervention. Delaying puberty allows individuals more time to explore their gender identity before deciding on more permanent interventions and prevents the physical changes associated with puberty.[6]

The preferred puberty-suppressing agent for both individuals assigned male at birth and individuals assigned female at birth is a GnRH Analogue.[6] This approach temporarily shuts down the Hypothalamic-Pituitary-Gonadal (HPG) Axis, which is responsible for the production of hormones (estrogen, testosterone) that cause the development of secondary sexual characteristics in puberty.[11] It is important for adolescents undergoing puberty-suppressing hormone therapy to be monitored by a medical professional.

Feminizing hormone therapy is typically used by gender-nonconforming persons, such as transgender women, who desire the development of feminine secondary sex characteristics. Individuals who identify as non-binary may also opt-in for feminizing hormone treatment to better align their body with their desired gender expression. [12] Feminizing hormone therapy usually includes medication to suppress testosterone production and induce feminization. Types of medications include estrogens, antiandrogens (testosterone blockers), and progestogens.[13] Most commonly, an estrogen is combined with an antiandrogen to suppress and block testosterone.[14] This allows for demasculinization and promotion of feminization and breast development. Estrogens are administered in various modalities including injection, transdermal patch, and oral tablets.[15]

The desired effects of feminizing hormone therapy focus on the development of feminine secondary sex characteristics. Transgender hormone therapy may limit fertility potential.[34] Should a transgender individual choose to undergo sex reassignment surgery, their fertility potential is lost completely.[35] Before starting any treatment, individuals may consider fertility issues and fertility preservation. Options include semen cryopreservation, oocyte cryopreservation, and ovarian tissue cryopreservation.[34][35]

A study due to be presented at ENDO 2019 (the Endocrine Society's conference) reportedly shows that even after one year of treatment with testosterone, a transgender man can preserve his fertility potential.[36]

Many providers use informed consent, whereby someone seeking hormone therapy can sign a statement of informed consent and begin treatment without much gatekeeping. For other providers, eligibility is determined using major diagnostic tools such as ICD-10 or the Diagnostic and Statistical Manual of Mental Disorders (DSM). Psychiatric conditions can commonly accompany or present similar to gender incongruence and gender dysphoria. For this reason, patients are assessed using DSM-5 criteria or ICD-10 criteria in addition to screening for psychiatric disorders. The Endocrine Society requires physicians that diagnose gender dysphoria and gender incongruence to be trained in psychiatric disorders with competency in ICD-10 and DSM-5. The healthcare provider should also obtain a thorough assessment of the patient's mental health and identify potential psychosocial factors that can affect therapy.[37]

The ICD-10 system requires that patients have a diagnosis of either transsexualism or gender identity disorder of childhood. The criteria for transsexualism include:[38]

Individuals cannot be diagnosed with transsexualism if their symptoms are believed to be a result of another mental disorder, or of a genetic or chromosomal abnormality.[citation needed]

For a child to be diagnosed with gender identity disorder of childhood under ICD-10 criteria, they must be pre-pubescent and have intense and persistent distress about being the opposite sex. The distress must be present for at least six months. The child must either:

Gender-affirming care is health care that affirms people to live authentically in their genders, no matter the gender they were assigned at birth or the path their gender affirmation (or transition) takes. It allows each person to seek only the changes or medical interventions they desire to affirm their own gender identity, and hormone therapy (HRT or gender-affirming hormone therapy) may be a part of that. Some transgender people choose to self-administer hormone replacement medications, often because doctors have too little experience in this area, or because no doctor is available. Others self-administer because their doctor will not prescribe hormones without a letter from a psychotherapist stating that the patient meets the diagnostic criteria and is making an informed decision to transition. Many therapists require at least three months of continuous psychotherapy and/or real-life experience before they will write such a letter. Because many individuals must pay for evaluation and care out-of-pocket, costs can be prohibitive.

Access to medication can be poor even where health care is provided free. In a patient survey conducted by the United Kingdom's National Health Service in 2008, 5% of respondents acknowledged resorting to self-medication, and 46% were dissatisfied with the amount of time it took to receive hormone therapy. The report concluded in part: "The NHS must provide a service that is easy to access so that vulnerable patients do not feel forced to turn to DIY remedies such as buying drugs online with all the risks that entails. Patients must be able to access professional help and advice so that they can make informed decisions about their care, whether they wish to take the NHS or private route without putting their health and indeed their lives in danger."[43] Self-administration of hormone replacement medications may have untoward health effects and risks.[44]

What happens to our bones as we age?

There are two major bone cell types, Russell says. The osteoclast, and the osteoblast. From the day that we are born, until around age 25 to 30, we make more bone than we lose. So, the bone strength gets better and better. And then somewhere around 25 and 30, we all sadly reach our peak bone density, and thats the strongest that our bones will ever be. Then, as we start the normal aging process, we start to lose more bone than we make and slowly the bone density begins to decline.

Men dont go through a menopause, Russell says. But they do have a slow lowering of their testosterone levels. We dont like to give elderly men TRT because it increases the risk of prostate cancer. sick, and you put them in a hospital bed, within about two weeks theyll start losing bone. The more that you can have activities where you are against gravity, the better your bone health is going to be.

How important is the relationship between muscle mass and strong healthy bones?

Muscle is attached to bone. Muscle pulls on bone. The stronger the muscle, the greater the pull on the bone, and the better it is for the bone, says Russell. So, if you have somebody that is really well defined, they have a nice cardio program, and a nice weight-strengthening program, then thats going to be beneficial. If you take a 40-year-old, who is a couch potato, and has some adiposity (fat) around their middle, they are not going to be as healthy as someone whos hitting the gym five days a week. Strong muscle helps bone, age is important, but for me, being a rheumatologist and seeing people of all ages, its really their fitness that speaks more than their age.

Russell points out that a good workout program becomes even more essential as we reach our 40s, because muscle mass is already beginning to decline naturally. You lose about a third of a pound of muscle mass per year, she says. You can work hard so that this doesnt happen. You can exercise and do your weight training, but if youre just a guy that doesnt really exercise you will start losing muscle and if you looked at an MRI of the that muscle, it has more fat interspersed with the muscle. For someone that is very fit, there is very little fat in the muscle shown on an MRI.

What should men avoid to maintain healthy bones?

A risk factor for osteoporosis is having more than three alcoholic beverages per day, says Russell. There are probably a good number of people that drink at least that much alcohol per day. Its not ideal for bone.

In addition, Russell adds that it is important to get checked out by a doctor because problems with bones can often be caused by other medical issues. Rheumatoid arthritis is associated with bone loss, she says. A lot of liver diseases are also associated with bone loss. If you have rheumatoid arthritis and you are treated with medication, you reduce the chances of bone loss. But some medications are associated with bone loss, like prednisone, or medications used to treat prostate cancer because they are trying to lower the testosterone level. Tobacco is also very bad for bone health. Tobacco is directly toxic to the osteoblast, the cell that makes bone.

So, its good to keep lifting in my 40s and beyond?

At any age, studying proper lifting form and not trying to move a force that is too heavy for you is a good idea to prevent injury, bone and joint problems. And in terms of minerals, Dr. Russell points out that vitamin D and calcium are essential for bone health, and these can

often be consumed through a normal diet, so make sure you include proper nutrition within your training resume. But when it comes to lifting weights, you still have plenty of life in those old bones!

A 41-year-old mother from Minnesota revealed that she lost more than 130 pounds in oneyeardue to consistency, some groupsupport and a concept called "habit stacking."

Sabrina Osland, of Chanhassen, started her weight loss journey after she was charged extra on a helicopter ride over a glacier with her son that was part of an Alaskan cruise, Today reported.

"If you were over a certain weight then you had to pay extra because apparently, it would be taking (a seat) away from somebody else. I dont know how 20 or 50 pounds really counts for a person. But really it was one of the most devastating things," Osland told the outlet.

The business sales consultant's weight fluctuated throughout adulthood, but she started to gain weight again after she gotmarried seven years ago.

The helicopter ride led Osland to start exercisingfor at least 30 minutes every day upon returning home. She set reminders on her phone and wrote a checklist of tasks to do every day, which included small changes such as drinking enough water, exercising and taking vitamins.

This "checks and balances" system would remind Osland about tasks if she failed to do them on time.

She later started walking for an hour or twice a day once her endurance improved. She then added strength training, but the COVID-19 pandemic hit, and she had to incorporate her new habits into her work-from-home life without a gym.

However, this presented Osland with an opportunity to change more things about her lifestyle as "all the excuses" she had before disappeared.

The mother started to change what she ate and switched her focus from "convenient" to "healthy" options. She began preparing fresh vegetables for snacks instead of potato chips and adding salads to dinner rather than pizza. The goal was to add protein and vegetables to the diet while attempting to take out as much of the carbs, Osland said.

Osland made these changes one at a time, and she would only add another until the last change became a habit. "It really was about taking those small steps," she explained.

This habit stacking "allowed her to develop healthy behaviors she can stick to easily," according to Today.

Aside from this, Osland started working out with a group called Alpha Strong after the lifting of COVID-19 restrictions allowed her to return to the gym. The group supposedly helped Osland bolster her strength training regimen and added accountability for her as well.

"The community in and of itself has just been amazing," Osland said, adding,"The friendships in the community that we have developed they hold you accountable.Developing that community and accountability has been huge.

Additionally, Osland said that progress was more important for her than perfection and that there was no problem if her workout in some days was not the best or if she did not eat as many vegetables as she should have in others.

"Its progress, not perfection. Thats my motto. If Im not doing something 100% then I feel like Im failing. And you just need to celebrate those successes along the way," Osland explained.

The mother, who has since gone down from 266 pounds to 135 poundsin a little over a year and is now in hermaintenance stage, pointed out that "it's not just about what you physically look like, but how much it impacts everything as a whole."

"There is so much more that comes with leading a healthy lifestyle," Osland said.

‘My 600-Lb. Life’ Lucas Higdon 2021 Update: Where Is He Now? – TV Shows Ace

My 600-Lb. Lifeintroduces us to a man named Lucas Higdon during tonights all-new episode and viewers are already wondering if there is a 2021 update on his weight loss progress and where he is now.

Warning: Spoilers ahead.

During tonights episode,My 600-Lb. Lifeviewers are introduced to Lucas Higdon. Lucas is 33-years-old. Initially, he admits he has no idea what his current weight is. He resides on his family compound in Conroe, Texas. Speaking to his sister Maegen, Lucas admits its been years since hes stepped on a scale. So, he had no idea how much he weighed. When Lucas Higdon finally weighs in for the first time during the episode, the scale reads 619 pounds.

Dr. Now is pretty quick to come for his sister hard. Speaking to the camera in the hall, Dr. Now speculates the family enables Lucas even if they dont realize they are doing so. Dr. Now believes Maegen enjoys having her brother as a babysitter and a homeschool teacher. So, she isnt super motivated to help him make changes to his current lifestyle.

As the episode progresses, Maegen seemingly confirms Dr. Nows fears when she admits her brother is losing weight too fast for her own comfort. She continues to explain him losing so much weight is going to force her to look elsewhere for a babysitter and homeschool teacher. Maegen admits she wants her brother to lose weight because it means hell live longer. But, she doesnt think shell find someone else who is as great with her kids.

Tonights episode ofMy 600-Lb. Lifeis a pretty happy one. Lucas Higdon admits he is in a bad place with his weight. But, he doesnt seem to let it stop him from getting up and moving around. Despite his weight, he is still able to do things such as taking care of two children and going grocery shopping. Lucas Higdon is able to meet his weight loss goals every step of the way with Dr. Now. Dr. Now admits Lucas should be losing a bit more than hes losing, but hes happy with the results overall. After a few visits and some dropped weight, Dr. Now approves him for weight loss surgery.

By the end of the episode, Lucas drops 218 pounds. He has reached a place where he can get a real job out of the house again. And, hes driving himself around. Considering how great his episode was and how committed he was to losing weightMy 600-Lb. Lifeviewers really want a 2021 update on him. Where is Lucas Higdon now?

After some deep diving, we discussed Lucas does have a Facebook page. Unfortunately, he doesnt have much in the way of public posts. He did update his profile picture back in October. He is wearing the same ski hat in the photo that he wore in much of his episode. His face looks much slimmer in the updated profile picture suggesting as of October he was still on track with his weight loss.

If that profile pic was snapped toward the end of filming and hes stayed on track, fans can speculate hes lost anywhere from 60-90 pounds since filming concluded. So, Lucas Higdon likely weighs somewhere between 310 and 340 at this point in his weight loss journey. His profile suggests he also make have picked up a job as a grocery delivery driver again.
