DEAR DR. ROACH: We are very fond of our family doctor. I was in to see him the other day, and it came up in conversation that he was going to be getting married.

We would like to acknowledge this event in his life with either a lovely card or a small gift. I know the old-school way of thinking is to never do things that are so personal when it comes to your physician. Do you think that still holds true today? N.W.

ANSWER: Physicians are human beings, and a thoughtful card is always appreciated. Gifts can sometimes be OK, but the American Medical Association has some guidelines for physicians on accepting gifts. I agree with them and you might find them helpful:

Be sensitive to the size of the gift: Inappropriately large gifts should not be accepted. Do not let a gift influence patient care.

Decline a gift that would present an emotional or financial hardship to the patients family.

Consider suggesting a charitable contribution in lieu of a gift.

I would add that sometimes rejecting a gift can be hurtful, and if accepting a gift conforms to the guidelines, I generally accept it with thanks.

DEAR DR. ROACH: Hi. Im a very healthy, 68-year-old male. My only issue is almost nonexistent testosterone. What type of doctor should I see? It seems to be a problem for many senior men. A.

ANSWER: Low testosterone is a concern for many men as they get older. In some cases, the concern is justified. Some men may have a problem making testosterone due to a condition in the brain (the hypothalamus and pituitary gland both need to be working properly for testosterone production) or in the testes, where testosterone is actually made. In these cases, testosterone replacement therapy is clearly indicated.

What is still controversial is whether to treat healthy older men who have a low testosterone level and no symptoms. Its not clear that treatment of low testosterone will prevent problems in the future.

Its possible thats the case, but I am very cautious about prescribing a drug with potential harms for someone without symptoms, when it has not been proven to be of benefit.

However, men with symptoms of low testosterone, such as low sex drive, depressed mood and loss of body hair, and who have clearly low testosterone levels may benefit from treatment, and a trial of testosterone treatment is appropriate.

Men should learn about potential risk (prostate cancer remains a concern, despite reassuring early studies) and have regular laboratory evaluation. Low energy by itself is not clearly a symptom of low testosterone and may not improve with testosterone treatment.

DR. ROACH WRITES: A recent column on easy bruising in older adults prompted several readers to recommend specialized makeup products to cover the discolored areas. That is not something I had considered, and I appreciate the recommendation. Another reader recommended dry skin brushing, but I could not find good evidence that this helps with skin discoloration.

"Tons of research supports this idea," says Hayim. "We used to rely on three square meals throughout the day, but more research shows us that eating small meals helps drop the weight easier. If you're following a low-carb diet to lose weight, you've probably figured out that not all foods low in carbohydrates are created equal.

In other words, eating a low-carb diet doesn't always provide fast and sustainable weight loss. If your version of low-carb includes large servings of high-fat animal products, processed meats and adding butter or coconut oil to your coffee, your food choices could be doing more harm than good.

When it comes to eating fewer carbs for weight loss, it's wise to focus on lean proteins and vegetables that are low in carbs and high in fiber to keep, which will keep you feeling full and provide other added benefits.

Here are some of the best low-carb foods for losing weight.

Salmon is an excellent zero-carb choice; it provides heart-healthy fats and brain-boosting omega-3 fatty acids. Omega-3s, which are a type of unsaturated fatty acid, may help decrease triglycerides, lower blood pressure, reduce blood clotting and decrease your risk of heart failure or stroke, per the Mayo Clinic.

A 3-ounce piece of cooked salmon packs nearly 22 grams of protein in 196 calories, per the USDA. Protein is the most satiating of the macronutrients (the other two being carbs and fat), so a protein-packed choice like salmon helps you feel full longer (without the saturated fat you'd get from proteins like red meat).

Chicken breast has zero carbohydrates and is an excellent source of protein. One medium chicken breast provides 193 calories and 36 grams of protein, per the USDA.

Research shows that protein plays a major role in weight loss. An April 2015 review in Clinical Nutrition found that eating a higher-protein diet containing between 1.2 to 1.6 grams of protein per kilogram of a person's body weight per day provided improvements in body weight and appetite.

A kilogram is equal to 2.2 pounds. So, a person who weighed 180 pounds, for example, would want to aim for between 98 and 130 grams of daily protein.

Too much math? There's a simpler solution: The analysis found that eating 25 to 30 grams of protein per meal provided similar effects.

Leafy greens like spinach, kale and arugula can add volume and flavor to your meals without adding many calories or carbohydrates. Leafy greens are a great pick because they allow you to add more food to meals to spark benefits, instead of taking food away.

Studies have found that doing just this adding voluminous veggies to meals can help people lose weight. When you break down the nutrition profile of greens, it's easy to see how they can provide so much good: One cup of raw spinach, for example, contains just 6 calories and less than 1 gram of carbohydrates with more than half of the carbohydrates coming from fiber, per the USDA.

Eggs are a versatile low-carb pick. For an easy grab-and-go option, hardboil a batch and eat a couple for breakfast or one as a snack. Don't forget to look beyond breakfast, too: Eggs can make a delicious addition to salads, avocado toasts and even zucchini noodles.

One large egg contains 70 calories, 6 grams of protein and less than 1 gram of carbohydrates, per the USDA.

Beyond protein, eggs are a healthy source of vitamin D, antioxidants and omega-3 fatty acids. Because of their stellar nutrition profile, they're a great option for vegetarians and meat-eaters alike.

Brussels sprouts are considered nonstarchy vegetables, per the American Diabetes Association. These veggies are naturally low in carbs and calories 8 grams of carbs and 39 calories per cup, per the USDA which makes them an excellent addition to any weight-loss plan.

Other examples of nonstarchy veggies include cauliflower, celery, carrots, cabbage and zucchini. Like leafy greens, these veggies can be piled on your plate to add volume; they'll increase your satiety and provide essential vitamins and minerals without adding many calories, carbs or fat.

Broccoli is a nutritional powerhouse, offering fiber and antioxidants with very few carbohydrates. One 3-ounce serving of broccoli contains 25 calories, 4 grams of carbs and 3 grams of fiber, per the USDA.

The majority of its carbohydrates are part of broccoli's fiber content, which make it a great choice. Fiber is the indigestible part of a plant food that doesn't get absorbed by the body, so it fills us up but doesn't stick around.

Enjoy this versatile veggie as a side or make it the meal's main attraction there is truly no wrong way!

Like chicken breast, roasted turkey breast is an excellent source of protein (26 grams in a 3-ounce serving, per the USDA) with zero carbohydrates and only 125 calories.

Swap in turkey for recipes that call for ground beef and you'll cut down on both calories and fat while still getting your protein fix.

When trying to lose weight, it's best to bake, broil, grill or steam lean sources of protein like turkey. This will help limit calories from fat that add up in cooking methods that rely on oil.

New dietitian: ‘Health is about more than just losing weight’ – Queensland Times

Stephanie Bennett's journey with health and wellbeing started simply with a love of food, and eating.

It was this passion for food, and a fascination with the human body that led her to the world of nutrition and dietetics.

Now, the dietitian has started a new job where she hopes to transform Toowoomba's view on what health and wellbeing really is.

After living and working on the Sunshine Coast, she made the move back to her home town to begin work at The Fit Lab.

Moving back to Toowoomba and working as a dietitian has presented a world of challenges and rewards.

"Some people come to me for weight loss, so if I can get them some sustainable weight loss, not just a fad diet, that's really rewarding," she said.

"Some people that have been suffering a lot with gastrointestinal upsets, so bloating, IBS symptoms - it really can affect their quality of life.

"Just making some small tweaks to their diet can make all the difference and improves their quality of life significantly."

Recent data from the chief health officer said Darling Downs residents are among the most unhealthy residents in Queensland.

The region ranked one of the worst in the state for their fruit and vegetable consumption, as well as their exercise rates.

This trend of unhealthy eating has been apparent to Mrs Bennett from day one.

"We've got an environmental factor playing on us a lot," she said.

"We have fast food, the activity culture is not as big here, the meal delivery services are around more lately.

"We've got a really busy lifestyle and people really struggle to implement healthy lifestyle changes.

"Eating is one part of it but addressing all the other parts of it, all of that needs to tie in together."

She said health and wellbeing could never come in a "one size fits all" mould - it's multifaceted.

"Health and wellbeing is finding your individual place where you're happy and content - it's your diet, and asleep, and exercise, and social interactions, and bringing health and wellbeing from that.

"Focusing on small, sustainable things you can add to the diet sort of kickstarts those health habits and kickstarts you on your journey to making healthy choices."

The asthma diet – The Hindu

With winter setting in and pollution levels soaring, people with lived experiences and health professionals tell us what foods may work and what may not, to help you breathe better

Like most kids his age, Aryaman, a teen living in Delhi, loves junk food. My son plays sport, but after a game hell pick up some packaged food from the shop in our society, says his mother . Within 15 minutes, hell start to wheeze or get a rash.

When it comes to asthma and wheezing, different people have different triggers. For Gurugram-based school teacher Lubna Seth, who developed asthma after moving to Delhi from Shimla a decade ago, its maida-based foods and anything cold. When shes very tempted, I microwave my bowl of curd or ice cream for 10-15 seconds before eating and follow it up with kadha chai or coffee.

Kalpana Balhara, a Delhi-based consultant with a child rights group knows she must avoid oily and spicy food, sweets, curd, and anything cold. Having developed asthma a few years ago and contracting COVID-19 recently, she values every breath she takes. But, as Lubna says, It is not always easy to give up certain foods.

Asthma is a lung condition in which the tubes that carry air to and from the lungs swell up or become narrow and cause breathlessness. The lining of the airways is always in a hypersensitive state characterised by redness and is easily affected by different types of allergens that can harm the lungs. Allopathic doctors say there is no cure, but treatment (medication) can help control the symptoms that worsen due to allergies to dust, pollution, pollen, tobacco smoke, chemical irritants and cold air. Emotional stress, obesity, and anxiety also trigger asthma in some people.

While people with a lived experience say that certain foods trigger reactions, research has so far only confirmed that artificial food colouring causes symptoms to escalate. Pune-based pulmonologist Dr Sundeep Salvi admits that research on lung health has not seen long-term studies. But those with asthma should definitely avoid all canned, processed and packaged foods as they contain preservatives, emulsifiers and food colouring agents. These are all chemicals and have a cumulative effect on the lungs and trigger wheezing, he says. Antioxidant-rich fruits and vegetables (such as aamla, blue berries, apple, papaya, guava, melons, oranges, grapes) can provide some protection, he says, but based on his clinical experience, Dr Salvi advises people to avoid certain fruits such as pineapple, banana, mango, , in order to control the symptoms and reduce suffering.

Dr Salvi advocates high water intake for healthy lungs. According to him, human beings require 10,000 litres of air every 24 hours to survive. When we breathe in air through the nose for it to reach the alveoli (the tiny air sacs in the lungs), the passageway needs to be 100% saturated with water. Those with asthma suffer from dry lungs and always need moist air for air passages to function without hindrance. Otherwise dry air can be an irritant and trigger not only a dry cough but also dry skin, he says. Asthma and eczema are related and in severe cases, if you suppress one, the other manifests.

The traditional forms of healing hold more answers for asthma though. Ramesh R Varier, senior physician at AVN Arogya Ayurvedic Hospital, Madurai, says, In Ayurveda, tamaka swasha (the equivalent of bronchial asthma) is related to indigestion caused by excessive kapha dosha aggravating foods. He agrees sweet fruits like mangoes and bananas are better avoided much like sour foods, both of which are considered good for the vata and not kapha dosha.

The time of eating is also important. Any dairy product or cold item had early in the morning or late in the evening may trigger respiratory distress, he says and also advises limited intake of black gram, beans, sesame and meat to avoid gas build up in the abdomen. Varier explains that food allergies and intolerances lead to the build-up of air or foodstuff causing bloating and shortness of breath. That is why Ayurveda recommends a diet that helps to reduce symptoms of irritable bowel syndrome and also treat eczema, through foods that help in elimination. Never overeat but follow your hunger because appetite is the appropriate precursor to nourishment, he says.

Pune-based Abhishek Devikar who practises naturopathy says mucus formation is the most important trigger for asthma. Pepper, ginger, basil, turmeric, jaggery are good immuno-stimulators for chronic diseases like asthma and when consumed with hot water or milk especially in winter, provide relief, he says.

Those with asthma have a hypersensitive mucus membrane (inner lining of the airways). Any allergen entering the body can cause the membrane to become dense, precipitating mucus production. This causes clogging of the air passage, leading to wheezing as the lungs go into spasms. In winter months especially, the mucus formation is more dense and consumption of anything cold, sour, starchy aggravates it. It starts with an immediate itchiness in throat, the first sign of allergic reaction, he says.

Overall, a diet rich in calcium, iron, iodine, protein, vitamin A, B, C and E, potassium, zinc are anti-inflammatory. They benefit the immune system and may help protect the lungs from damage due to smoking and air pollution, says Manjari Chandra, a functional nutritionist and author of the book Eat Up, Clean Up: Your Personal Journey to a Healthy Life.

People are now waking up to improving the quality of their lives because treating asthma is also largely individual-specific. You need to understand how your body reacts to environmental changes and takes to different foods. By trial and error, and by a process of elimination in the diet or otherwise, people are following what suits them the best, says Manjari.

She adds that everyone can make their own list of foods that heal (including fibrous green leafy vegetables, white and red radish, turnips, leeks, ginger, spring onions and bitter gourd, onion, sweet potatoes, unpolished millets like jowar and bajra, ferments such as idli, dosa and appam, nuts and seeds). Foods that hurt include all forms of white sugar, salt, refined foods like flour, oil, white rice, bread, cheese, pasta, potato and yam.

If you have asthma, you know cold, dry weather can be troublesome, especially as you may be susceptible to secondary infections. So you up the intake of your immunity boosters with antioxidant rich foods.

Eating lots of sugar may damage your colon – Villages-News

A study from the University of Texas, Southwestern Medical Center, found that mice fed diets high in sugar developed severe colitis by increasing harmful colon bacteria and decreasing healthful colon bacteria (Sci Transl Med, Oct 28, 2020;12(567):eaay6218). The researchers fed mice various dietary sugars such as glucose, fructose, and sucrose for seven days. The high-sugar diet damaged the guts protective mucus layer to increase risk for persistent diarrhea, abdominal pain, and rectal bleeding. Harmful bacteria, such as Akkermansia, produce enzymes that break down the mucus that lines and protects your colon from invasion by other bacteria, while the healthful bacteria such as lactobacillus and Bacteroides fragilis markedly increase this mucus protective layer.

When the researchers fed feces from the sugar-treated mice to mice that had not received the sugar diet, they also developed the same changes. This suggests that the intestinal damage was caused primarily by the increased growth of harmful bacteria caused by the high-sugar diet. I find this very disturbing because since the 1960s, the food industry has added high-fructose corn syrup and other sugars to all sorts of foods and drinks to make them taste better. During this period, the incidence of colitis, Crohns disease, and irritable bowel disease have increased significantly. Many other studies show that a high sugar diet also increases risk for heart attacks and certain cancers by the same mechanism (JAMA Intern Med, 2014;174(4):516-524), and the chronic intestinal inflammation of Crohns disease and ulcerative colitis is associated with increased risk for diabetes (Clinical Gastroenterology and Hepatology, Aug 14, 2019).

How Sugar Damages Your IntestinesMore than 100 trillion bacteria live in your colon, and these bacteria help to govern your immune system. The healthful bacteria are happy eating the food that reaches them in your colon, while the harmful bacteria are not happy with the food that you eat and instead try to invade the cells lining your colon. Your immune system tries to defend you by producing huge amounts of white blood cells and chemicals that work to destroy the invading bacteria by punching holes in their outer membranes and trying to kill and eat them. The damage caused by invading colon bacteria turns on your immune system to causeinflammation(PLoS One, Apr 13, 2017:12(4):e0176062). The good and bad bacteria compete for space in your colon.

Sugar in foods and drinks is supposed to be absorbed in the upper intestinal tract, but taking in large amounts of sugar can cause some of the sugar to pass through the intestines unabsorbed. This sugar arrives in your colon where it can harm you by keeping healthful bacteria from growing in your colon and encouraging the overgrowth of harmful bacteria (PNAS, Dec 17, 2018). Specifically, unabsorbed sugar in your colon can prevent the good bacteria from producing a key protein called Roc (regulator of colonization), which is required for growth of the healthful species Bacteroides thetaiotaomicron (B. theta) in your colon. Without Roc, the B. theta do not thrive and multiply, which allows harmful bacteria to replace them in your colon. The researchers demonstrated this by engineering a strain of B. theta in which sugar did not suppress Roc, and showed that these engineered bacteria were able to thrive in the colons of mice fed a high-sugar diet. If these results can be duplicated in humans, they will show how eating large amounts of sugar can affect the numbers of good and bad bacteria that grow in your colon.

My RecommendationsThe types of bacteria in your colon are determined primarily by your lifestyle and what you eat (Nature, March 8, 2018;555:210215). Sugar is supposed to be absorbed in your intestines before it reaches your colon, but if you take in so much sugar that it overloads and reaches your colon, it will cause harmful bacteria to overgrow there. I recommend limiting all sources of added sugars in your diet, particularly all drinks with sugar in them.

Read the list of ingredients on any new food you buy and realize that you are getting sugar if you see any of these terms: anhydrous dextrose, brown sugar, cane juice, confectioners powdered sugar, corn syrup, corn syrup solids, crystal dextrose, dextrose, evaporated corn sweetener, fructose, fruit nectar, galactose, high-fructose corn syrup (HFCS), glucose, honey, invert sugar, lactose, malt syrup, maltose, maple syrup, molasses, nectars, pancake syrup, raw sugar, sugar cane juice, sucrose, and so forth. Sugars extracted from fruits (such as grapes or apples) are no more healthful than any other source of sugar.
