UIHC performing trial to reduce celiac disease symptoms – UI The Daily Iowan

Posted: September 25, 2020 at 12:58 pm

A team of doctors and researchers at UIHC are conducting a study that has the potential to reduce common side effects found within those with celiac disease, one of the first hospitals to do so.

David Elliott is currently investigating a trial drug that could help digestion for those with celiac disease, which causes an immune reaction to eating gluten.

Elliott, Director of the Division of Gastroenterology-Hepatology at the University of Iowa Hospitals and Clinics, said his team is studying larazotide a compound known to decrease intestinal permeability to see if will assist patients with celiac disease who are on a gluten-free diet.

Most people with celiac disease, when they go on a gluten-free diet feel wonderful, and dont have problems, but there are these small subset of individuals that still have some [gastrointestinal] complaints, even though theyre on a gluten free diet, Elliott said. Thats where this is targeted: for those people who have more difficulty with a loose tight junction This drug may help that.

Elliott, who is also a professor of internal medicine, said the team is actively recruiting participants for this study. One of the main requirements for participants is that they must have a positive serology and positive biopsy for celiac disease within the past six months to a year.

It is ideal if prospective participants experience ongoing symptoms associated with celiac disease, Elliott said, such as bloating, abdominal pain, abdominal cramps, and gas at least once a week.

Elliott said UIHC is one of the first hospitals to work directly with larazotide. While it has been studied in the past, this particular trial is in phase three of four, making it rather advanced.

Jessica Valestin, clinical research manager in Elliots division, said the research group is optimistic about the trials potential outcomes and how it will affect individuals with celiac disease.

Our group chose this study as we believe in helping others and moving science forward, Valestin. We were very excited at the chance to potentially be able to help patients suffering with celiac disease.

Elliott said the drug will be taken as a pill, and will help patients determine what is directly causing their symptoms and how they can reduce them.

If this pill helps, then its something we can give people, and if it removes their symptoms say, Oh no youre doing fine, youre gluten free the problem that youre having is because of this, Elliott said. Then they can decide if they want to stay on the medicine or whether they want to go off the medicine.

He said in previous trials, it was believed that participants could eat gluten while taking the drug, as it walls off gluten and allows them to return to a more common diet.

This previous trial was not successful because the symptoms of celiac disease returned. However, the team discovered patients discomfort associated with celiac disease was reduced while on a gluten-free diet while also taking larazotide, he said.

The discovery initiated the current trial to see whether the drug helps tighten the gut, decreasing the amount of gluten traveling between the intestinal cell wall, rather than through it. If so, Elliott said, it will aid in lessening the discomfort experienced with celiac disease.

RELATED: University of Iowa Hospitals and Clinics seeing sicker patients

UI senior Ethan Upchurch said he was diagnosed with celiac disease in 2016, and currently serves as the community outreach coordinator for the UI Celi-Yaks Club.

The Celi-Yaks aim to decrease the challenges of navigating the college food scene and promote advocacy and awareness of celiac disease and other forms of gluten sensitivity.

Upchurch said the goal of the current investigative trial corresponds with the clubs goals.

Our mission statement sort of lies in the same vein as the study does were just trying to approach [celiac disease] from a different angle, Upchurch said.

Upchurch has participated in past studies and is currently in the process of applying and being accepted for the current clinical trial conducted by Elliott.

I had completed one about a year ago, and to be honest, on top of the benefits of just promoting celiac research, the compensation is really great, Upchurch said.

Upchurch said he received $1,500 for participating in a former study regarding celiac disease. He said he feels more comfortable participating in the current trial as it involves routinely taking a pill, rather than using injections of a specific compound on a daily basis something he had to do in the previous trial he participated in.

It did seem a bit less invasive than the other one, he said. The initial study that I had done, that involved giving myself injections in my abdominal area, and that was just a bit much for me, it wasnt super painful or anything, it was just kind of annoying to have to stay regiment with that and consistently give myself shots in the abdomen every day.

Upchurch said this research gives him hope for the future of celiac disease and the symptoms associated with it.

Its refreshing to see that theres research thats being done out there, the senior said.

Valestin said their research group aims to positively impact patients and initiate the development of medication that will help those living with celiac disease.

Our group hopes to improve patients overall quality of life, especially those who live with a chronic illness, said Valestin. Clinical trials offer hope for better medications for the future.

Its quite understandable that after months of staying at home, working from home, and even working out at home, you may want to return to the gym to meet your weight loss or fitness goals. Gyms in India had been allowed to reopen, thanks to the Unlock 3.0 regulations, but the question that still remains is, how safe is it to return there?

India ranks second in the list of nations with the most number of confirmed COVID-19 infections, third in the list of global deaths, and the pandemic is far from over. The risks of transmission in crowded places and the need for stringent precautions still remain the same, especially in a place like a gym. A recent study in the Journal of Allergy and Clinical Immunology points out that although gyms and workout classes provide a sense of community, support, and personalized instruction, they are also crowded areas with lots of surface area that can transmit infections, and simply wiping sweat off with a towel is not enough.

So, while you may want to rejoin your gym, there are quite a few considerations involved before you actually do. Keep the following tips in mind if you intend to return to your gym while keeping COVID-19 infection at bay.

1. Call your gym: Its necessary that you call your gym to find out what precautions they are taking against COVID-19 infections. Ask about how they intend to manage hygiene, temperature checks, social distancing, staff and member health and if frequent disinfection of the shared gym equipment is being done. These are basic requirements and if your gym isnt providing them, then rethink your decision or choose a gym that does.

2. Check location: Before you head out to the gym, check if the locality or building where the gym is situated has had a surge of infections lately. This can be easily done via a reliable app, news agency or by asking your municipal representatives.

3. Prefer ventilation: If your gym is outdoors or has proper ventilation, then it may be safer than other gyms that are indoors or located in basements. Indoor spaces which are not ventilated properly tend to be breeding grounds for bacteria, viruses and fungi of all kinds, and therefore the risk of infection is much higher there.

4. Stick to social distancing: While wearing a mask constantly while exercising is neither possible nor advisable, social distancing while youre in the gym is your best bet on safety when you return to the gym. Avoid rush hour and opt for staggered timings with your trainer.

5. Avoid sharing: Carry your own gear and avoid using shared equipment, especially foam rollers and exercise balls, as much as possible. This means staying away from cycling machines, treadmills, dumbbells, barbells and weighted plates.

6. Sanitize: Carry hand sanitizers, disinfectant wipes, sanitizer sprays, disposable tissues or towels and use them properly. This will come in handy especially if youre using shared equipment. Use the sanitizers to wipe down any shared equipment both before and after using.

7. Assess your health: If you are feeling unwell due to any reason, dont go to the gym and stay at home until you get better. Whether you have COVID-19 or not, stepping out while youre sick also endangers others, and this is especially true in the case of closed spaces like a gym or a workplace. So, ensure your safety as well as that of others, and ask your fellow gym members and management to do the same.

TikTok is cracking down on weight loss ads that promote harmful body images – The Verge

Posted: September 25, 2020 at 12:55 pm

TikTok is putting new restrictions on weight loss ads as the app increasingly comes under criticism for promoting dangerous diets. The new policy bans ads for fasting apps and weight loss supplements. It also puts increased restrictions on other weight loss-related ads, like limiting ads for weight management products to users over 18 years old and not allowing those ads to promote a negative body image or negative relationship with food.

Some of the new restrictions are vague, but theyre broadly designed to limit TikTok users especially teenagers exposure to potentially harmful imagery and language. These types of ads do not support the positive, inclusive, and safe experience we strive for on TikTok, Tara Wadhwa, TikToks safety policy manager, wrote in a blog post this morning.

TikTok has been criticized this year for running both ads and user-created videos that seem to promote eating disorders or otherwise unhealthy diets. BuzzFeed News reported in February that the app was filled with pro-eating disorder content that was triggering to some viewers. In July, the New York Post wrote about the dangers behind the apps what I eat in a day trend, in which people sometimes show themselves eating very little. Rolling Stone reported that TikTok was advertising dangerous fasting diets to teenage girls.

While the advertising crackdown is a useful starting point, the real thing TikTok has to contend with is its much-lauded algorithm. The apps For You Page endlessly serves up videos based on what gets people to stick around, so users could find themselves watching harmful videos without actively seeking them out. TikTok notes that you can long-press on a video and select not interested to tell the app to stop surfacing similar videos if you come across one.

TikTok is also partnering with the National Eating Disorder Association and will start redirecting searches and hashtags for terms it deems unsafe to the associations helpline.

TikTok wins the hearts of people once again by banning weight loss ads – Digital Information World

Posted: September 25, 2020 at 12:55 pm

Social media marketing has gotten really common these days. Businesses and advertisers never miss an opportunity, to market their products and services. These days a number of advertisements show things like how to lose weight, weight loss pills and weight-reducing exercises. One of the platforms where this type of advertisements are getting really common is TikTok.

The purpose of these advertisements is to body shame people and attract them towards buying their products. These type of advertisements are dangerous for users and might harm the youngster.

In order to protect TikTok users, The CEO of TikTok Zhang Yiming introduced new policies which banned advertisements of products which might be unhealthy for the public. As per Yiming, these Advertisements make TikTok an unsafe platform for its users.

Sometimes companies like TikTok do not have complete access to the advertisements shown on their app, hence TikTok cannot control the advertisements which promote weight loss supplements, but they can try that these advertisements are not shown to people who are under 18. As these advertisements affect the brains of the minors.

In case a TikTok user comes across any of the harmful advertisements they can block those advertisements and report them. Once the report reaches the TikTok employees, they will have a look at it, and if the advertisement will be against the app's policies it will be immediately removed from the app.

The main reason why TikTok took immediate action on these type of advertisements is that; TikTok is used by people of every age and mostly Tiktokers lie between the age of 16-24 and these type of advertisements might be harmful to their health.

These type of adds are not only shown on TikTok but on almost all the social media platforms. But as apps like Facebook have a higher rate of older people so these advertisements may not be very risky for Facebook users. Similarly, a few years back Instagram also banned these type of advertisements, but they were not very successful in doing it.

As the days are passing the number of these shameful adds are also increasing and the content of these apps are becoming more dangerous for the users.

In 2018 a trend of Snapchat dysmorphia became very famous amongst the young generation. This trend supported plastic surgeries and made people look like a snap chat filter in real life. Companies marketed these products through Instagram and TikTok. At that time TikTok owners were not able to control the spread of these apps but now TikTok has taken a great step by banning these apps.

Unexplained weight loss must be investigated | Lewiston Sun Journal – The Bethel Citizen

Posted: September 25, 2020 at 12:55 pm

DEAR DR. ROACH: I am a 72-year-old man in good health. I have lost about 12 pounds in the past year, with no explanation. Im 5 feet, 9 1/2 inches tall, and my morning weight has dropped from 166-168 pounds to 154-156 pounds. It is still slowly falling.

I eat well, and Ive exercised an hour every other day for 36 years. Ive had on-and-off atrial fibrillation for 14 years, with two ablations, but since I began flecainide two years ago, no more episodes. I also take 10 mg simvastatin and 1,000 mg vitamin D3 daily.

My doctor did bloodwork and ordered a full CT scan, but found nothing. Since the pandemic began, Ive been walking 3 miles every other day, alternating with my days on the stationary bike at home. The doctor thinks I may be burning more calories than Im eating, but I doubt it.

Would you have any ideas? I was a skinny child, and I weighed just 126 pounds or so into my mid-20s, until I got married. So maybe Im just returning to the high basal metabolism of my youth. D.S.

ANSWER: Unexplained weight loss should be taken seriously, especially in an older person. Getting bloodwork and a CT scan is a reasonable start, but there are some additional issues that should be considered.

Your doctor is right to think about energy intake as well as energy expenditure. The first to do is to see how many calories you are eating. A careful food diary can accomplish this. People with major depression and other neurologic and psychiatric illnesses sometimes eat very little, and these should be carefully explored.

Sometimes, the body cannot absorb calories properly. Celiac disease and inflammatory bowel disease are the most common, but there are others. The body may lose calories; diabetes causes sugar to come out through the urine. Some infections can cause profound weight loss, such as tuberculosis consumption is an old name for this disease HIV, viral hepatitis and parasites.

Your metabolism might indeed be high, but that is not a normal condition of aging. Overactive thyroid disease is a condition your doctor has probably already looked for.

Medications are an uncommon cause of weight loss. Flecainide has been reported to cause weight loss in 1% to 3% of users. Some supplements can cause weight loss, too.

Cancer leaps to the mind for many when they hear about weight loss. Colon cancer and prostate cancer should be considered in any 72-year-old man, but especially with weight loss. The CT scan, if it included chest, abdomen and pelvis, should have found lung, kidney or pancreatic cancers, which all are frequently associated with weight loss.

Since your weight loss continues, its time for a more thorough re-evaluation. Your youthful basal metabolism probably hasnt just returned.

DEAR DR. ROACH: I am a small-boned 80-year-old woman who was just advised to begin osteoporosis treatments. At age 80, will the medication actually slow down my bone loss, or am I too old to see any better results from next years bone density test? Im very active, ride an exercise bike 3 miles and walk a treadmill 1 mile at least five days a week. I also take four daily tablets of Citracal+D3 tablets. S.M.

ANSWER: You are not too old to benefit from treatment of osteoporosis. Plus, the exercise you do will certainly continue to help your bones and the rest of you.

Bone density changes slowly. It may take two years to be sure of the benefits, but that doesnt mean you arent getting benefits as soon as you start the medicine.

Cleveland Clinic Study Identifies Weight-Loss Threshold for Cardiovascular and Survival Benefits in Patients with Obesity and Diabetes – Newswise

Posted: September 25, 2020 at 12:55 pm

Newswise Wednesday, September 23rd, 2020, CLEVELAND: A Cleveland Clinic study shows that 5 to 10 percent of surgically induced weight loss is associated with improved life expectancy and cardiovascular health. In comparison, about 20 percent weight loss is necessary to observe similar benefits with a non-surgical treatment. The findings also show that metabolic surgery may contribute health benefits that are independent of weight loss. The study is published in the October issue of Annals of Surgery.

This large observational study looked at 7,201 Cleveland Clinic patients: 1,223 patients with obesity and type 2 diabetes who underwent metabolic surgery (bariatric or weight loss surgery) were matched to 5,978 patients who received usual medical care. About 80 percent of the patients had hypertension, 74 percent had dyslipidemia (elevated triglycerides and cholesterol), and 31 percent were taking insulin to treat their diabetes.

Using different statistical models, the effects of weight loss were studied to identify the minimum weight loss needed to decrease the risk of death and of experiencing major adverse cardiovascular events, such as coronary artery events, cerebrovascular events, heart failure, kidney disease, and atrial fibrillation.

Following metabolic surgery, the risk of death and major heart complications appears to decrease after about 5 percent and 10 percent weight loss, respectively. Whereas, in the nonsurgical group, both the risk of death and major cardiovascular complications decreased after losing approximately 20 percent of body weight, said Ali Aminian, M.D., director of Cleveland Clinics Bariatric & Metabolic Institute, and lead author of the study.

This study suggests greater heart disease benefits are achieved with less weight loss following metabolic surgery than medical weight loss using lifestyle interventions. The study findings suggest that there are important benefits of metabolic surgery independent of the weight loss achieved, said Steven Nissen, M.D., Chief Academic Officer of the Heart, Vascular & Thoracic Institute at Cleveland Clinic, and the studys senior author.

The groundbreaking STAMPEDE study showed metabolic surgerys beneficial effects on blood glucose control. Since then, additional studies have observed health benefits other than weight loss following metabolic surgery. In fact, this research is a secondary analysis of a large study that showed weight-loss surgery is associated with a 40 percent reduction in risk of death and heart complications in patients with type 2 diabetes and obesity.

Researchers continue to study the physiological changes in the surgically modified gastrointestinal tract, the impact on hormone secretion and the microbiome. Those beneficial changes may contribute to the cardiovascular and survival benefits of metabolic surgery, independent of weight loss. More research is needed to better understand the underlying mechanisms for the health benefits of metabolic surgery in patients who have obesity and type 2 diabetes.
