Why Men Are Turning to Cosmetic Procedures for a Competitive Edge in the Boardroom – Robb Report

Posted: August 17, 2020 at 4:00 pm

Not long ago, a successful Silicon Valley tech entrepreneur decided to make a risky new investment hed been toying with for years. Id just hit 50 and sold my second company, he recalls. I looked at myself and thought, I have another 20 years of work in front of me, so Im going to go do this now.

Marc paid $25,000 for a lower face-lift and a nose job. (His name and those of the other patients who spoke to Robb Report have been changed at their request.) One of the best investments Ive made, he says. Unlike other investments in Marcs career, this one was a closely guarded secret, known only to his doctor, wife, brotherand now you.

Nine out of 10 cosmetic procedures in the US are performed on women. Yet to Marc and a growing number of high-flying men, nips, tucks and injections have become stealth weapons to deploy in a Darwinian battle for corporate survival. I play in the high-tech and start-up world, where older individuals will be passed over, says Marc, who also got his first Botox shots this year. The software wars take a lot of energy and commitment. I simply aligned my outer appearance with my inner perspective.

Its not just about advantage in a youth-oriented workplace. As gender roles evolve, vanity is losing much of its stigma for men in general. The beauty buffet, once ladies-only, is now open to all, with men increasingly moving from the hors doeuvres (grooming, beard cultivation, skin care, dieting and exercising) to the appetizers (cosmetic dentistry, hair replacement, hormone therapy) and on to the entrees (Botox, fillers, non-invasive fat reduction) before ordering up the piece de resistance: plastic surgery. Chanel, Fenty and Tom Ford now also offer a once-unthinkable side dish: makeup for men, leading them into the realm of foundation and eyebrow gel.

Tom Fords makeup for men.Tom Ford

Men dont use the word beauty, of course. Women talk about beauty. Men talk about vitality, virility, competitive edgethats a masculine way of describing what is essentially vanity, says William Liu, professor of psychology at the University of Maryland, who specializes in issues around masculinity. But what they are really talking about is warding off existential anxiety around death.

The idea of stigma has changed, says Marc. Women should not always have to look made-up, and men can wear makeup. Those mores are changing. Theres still a little bit of raising an eyebrow right now, but its becoming much more acceptable. Some people like to spend money on expensive cars. I like to spend money on myselfI consider my body to be the vehicle I drive in.

For some, cosmetic procedures fit smoothly into the narrative of personal empowerment and the growing idea that you can create yourself: your gender, your face, your identity. Its the old American idea of self-improvement, sliced and diced for the 21st century. Behavior that would once have seemed narcissistic has been reframed as an act of self-care. For starters, technology means theres no longer any need to tolerate physical imperfection. Plus, while millennials are accepting of quirky traits and would be loath to admit to holding a single ideal aesthetic, they also consider the very notion of judging other peoples lifestyle choices so old-fashioned that any taboo surrounding cosmetic enhancements seems like a relic from another era. So lets all get lip fillers.

And they will: Millennials are nearly twice as likely as people over 35 to be considering a cosmetic procedure in the next year, according to a poll last year by RealSelf, a cosmetic-surgery review website. The trend is toward transparency and removal of stigma for everything to do with self-care and self-love, says Simon Ourian, M.D., whom Kylie Jenner has credited forher signature pout with Juvederm, an investment on which Jenner has since built a nine-figure cosmetics business.

More cosmetic work is done in the United States than in any other country, according to a study by the International Society of Aesthetic Plastic Surgery. In 2018, the last year for which figures are available, the American Society of Plastic Surgeons (ASPS) reports that Americans spent a record $16.5 billion on plastic surgery and its twin progeny, Botox and fillers. Botox and other botulinum-toxin brands accounted for 42 percent of the 17.7 million procedures performed in the US by plastic surgeons that year. The real number of such injections is far higher, because they are also routinely dispensed by dentists, dermatologists, ophthalmologists and even walk-in beauty bars.

Adobe

More men are being seduced by these speedy pick-me-ups as the barriers to entry, both practical and social, diminish. In the US, male use of fillers and Botox has risen by 101 percent and 381 percent, respectively, since 2000, according to the ASPS, while the number of surgical nose jobs has dropped by 65 percent. Demand for handsome noses has not declined: Its just that many men now have their noses adjusted with less daunting fillers instead, according to Alan Matarasso, M.D., a plastic surgeon on New Yorks Upper East Side.

All the leading cosmetic surgeons and dermatologists interviewed for this article say men account for 20 to 35 percent of their clientele, a proportion that is growing while also broadening in scope. As with most style trends, gay men were the first to catch on. Then came the metrosexual type that would come in for Botox after their manicure, as David Mabrie, M.D., a facial plastic surgeon in San Francisco, recounts. Now its more mainstream guys. Guys guys. Men who a few years ago would have considered it a burden to trim a four-inch hair growing out of their ear are now smoothing their fine lines and filling their nasolabial folds.

In New York, the male clientele for Gerald Imber, M.D., has changed from largely those in entertainment to 95 percent CEOs and similarly accomplished professionals. These guys spend more on jet fuel coming to New York than they do on me, he says. Fifteen years ago, says Ourian, his Los Angeles patients were likely to be in entertainment, but the trend has completely changed and they are now from all walks of life. He adds that his practice includes ultra-wealthy businessmen, politicians, heads of state, three kings and five queens.

Even so, enough residual stigma remains that most men keep their Botox habits secret. And for some, the thrill of a clandestine hobby is part of the appeal. Marc likens it to membership in an exclusive club. One of Imbers patients, a 67-year-old real-estate developer from New Jersey, whom well call Nick, also relishes being a member of a secret elite club, in which the members are unknown even to each other. No one knows about it, he says. Thats just my masculine insecurityits a vanity issue. I work a lot with the construction industry, and no one there goes into work and says, I just had a face-lift.

Adobe

Nick felt that his jowly, hangdog face didnt match his gym-honed physique. I thought, If I cut my head off, I would look a lot younger, he says with a chuckle. But before I do that, Ill see if Dr. Imber can help. So three years ago, he took his girlfriend, whos 20 years his junior, for some couples liposuctionthat was the test runand followed up with a facelift.

I checked into the Carlyle, stayed three days, took the wrappings off and went home, Nick recalls. When I went out, I ran into people I knew at a restaurant. They asked if Id been on vacation. He describes with delight how he was asked for ID at a senior citizens lockdown shopping hour at a grocery store in the Hamptons.

Age has become elastic. On the one hand, Nick does not consider himself old at 67, but in youth-obsessed Silicon Valley, 35 is thought over the hill. There, the surgery is starting younger and younger, as tech-industry executives, who are mostly male, strive to appear relevant to their millennial overlords.

Larry Fan, M.D., a San Francisco plastic surgeon, says his clients now describe a work culture where if theyre over 30 they feel less relevant. Their work involves providing services to younger consumers, and tech luminaries are very young. My patients who are in their 50s say, Im the old guy in the room, and I dont want to feel that way. Young people have a kind of OK boomer mentality.

Fan says his male patients ask to look like Elon Musk: Hes aspirational for his work and [looks like he] has had some enhancements done, hair transplants and other things. He looks like a normal person but masculine and chiseled.

Elon MuskAP

Some of Fans patients, whom he describes as senior tech billionaires with famous names, regard cosmetic procedures as a form of bio-hacking (see here). Their mentality is: I want to live forever if I can find a way, Fan says. I believe in technology, and part of that is new treatments that help me have fewer wrinkles.

Ageism in the tech industry is not confined to Silicon Valley, according to one recruitment executive who worked with software start-ups in New York. The hiring managers were in their 30s, he explains. There are laws against saying youre looking for someone young, so instead they would say we need a culture fit. That was the number-one most important factor. Theyre looking for someone to grab a beer with. The recruiter put it down to the hubris that comes with gaining a lot of power at an early age. It means that they dont think they can learn anything from an older person. They feel invincible.

George, 53, the CEO of a large software company in the Bay Area, agrees that a youthful demeanor is a prerequisite in tech. Whatever HR says, you do look at pictures when you hire someone, and you look at their digital footprint, he says. Are they the right cultural fit? Do they take care of themselves? For better or worse, when you hire someone for a high-stress, high-performance job, part of that is their appearance.

In 2010, five years after Georges divorce from his first wife, his daughters took charge of his online dating profile. Being millennials, they said you have to look great online, he recalls. I realized that pictures had become very important, not just from a dating perspective but also on LinkedIn. That was a turning point for me.

He had dabbled in Botox but, with that revelation, became a devotee. A dermatologist who I played golf with said, If you want to stay looking like that, come in. I kept it up until I was 50, then I added fillers once a year, to keep the volume of my face, George says. Last year, he went to Sachin Parikh, M.D., a Palo Alto surgeon, for a hair transplant. George keeps the procedures private, partly as a career strategy. I dont want to talk about it because its still perceived as less manly. Also, men dont like to share tricks that may make them more competitive in the market. I do cryotherapy, I ride the Peloton every day and Ive had all the genetic testing to see what [conditions] I am predisposed to have, what foods do I process well. I see it all on the same spectrum as bio-hacking.

Adobe

The paradox of a man being furtive about a procedure he claims is akin to watching his sugar intake is surprisingly common. Talking about going to the gym is OK. Talking about getting a haircut is OK, says Imber. But cosmetic procedures are something they will do for themselves only, and they do not talk about it. In a culture of secrecy, it can be hard for surgeons to attract male clients through word of mouth. But once men are through the door, they keep coming back. Parikh describes the typical profile: The majority have dabbled in teeth whitening and skin care, and they work their way up the ladder from collagen stimulating treatments to fillers and Botox.

Hair-loss consultations can also be a gateway drug to other treatments, says Jessie Cheung, M.D., a Chicago-area dermatologist. A man will typically come in to talk about losing his hair, and we check for hormone deficiencies, she says. If their testosterone is not optimized, I will explain the benefits, and of course they want to perform better. Most clients who come for hormone therapy often get cosmetic treatments, too, and vice versa.

Other in-demand body treatments for men include CoolSculpting, a form of nonsurgical fat removal using cryotherapywildly popular, according to Fan, whose clientele for the procedure is 50 percent maleand skin tightening using ultrasound (Ultherapy) or radio frequency (FaceTite). But injectables are the money-spinner: a 12-year bull run of year-on-year double-digit growth, says Parikh.

Adobe

Paul Nassif, M.D., a facial surgeon famous for his TV appearances fixing bad surgery on the E! News show Botched, believes men are becoming more susceptible to the pressures of Instagram perfection. They are now on the same track as women, he says. Eight years ago, men were using Botox for work. Now I would say they are trying to make themselves look better on social media.

With fillers they can look like a filtered photo. We call it selfie dysmorphia. One of the biggest cosmetic trends of last year was the growth of reversal treatments for faulty fillers and surgical procedures, with liposuction revision up by 183 percent, according to RealSelf.

In New York, Matarasso is expecting a flood of business as soon as lockdown restrictions ease, after months of financiers, lawyers and such inspecting their double chins on Zoom. He believes that the crazy unemployment rate will lead to more treatments as job applicants grow desperate to gain an edge over their peers.

As demand for cosmetic enhancement rises, any remaining stigma, even among the middle-aged, seems likely to die away. When I talk about it with my close friends, I find a lot of them do it, too, says George, the software CEO. Its dont ask, dont tell, but when you do ask, they do tell.

Four PA women’s weight loss journeys inspire others to "Murder the Scale" – WPVI-TV

Posted: August 17, 2020 at 3:59 pm

BRISTOL, Pa. -- Four lovely women in their forties are teaming up against a common threat: their weight scales.

The colorful cast of lifelong friends have lost hundreds of pounds combined over the last decade. But the COVID-19 pandemic served as a wakeup call for quarantine calories creeping up.

"There's days when I don't feel like getting up. There's days when I don't want to work out," said Siretta "Ree" Lockett, one of the founding members. "But when you're held accountable, and you got three other people in your ear saying you can do this, you got this, it's just amazing."

The pound-killing teammates have been friends since their childhoods. Growing up and experiencing similar struggles, they always find a way back to each other.

"Although we live close together, we kind of live our own separate lives," said Kwesi "Kay" Hutchings-Williams. "But when we come together, and we see each other, and know that we're coming for a certain goal, we're all uplifted."

That camaraderie electrifies their work-out atmosphere, no matter the place, time, or weather. They often live stream their workouts to Facebook and demonstrate a low-impact version of their routines so group members can participate from home.

Lorriana "Lou" Dix was 330 pounds in 2009. Like some of her friends, she received a surgical procedure to dramatically cut her weight.

"All that resistance and stuff that you come against, you have to push past it," Lou said, referring to the temptation of her delicious cooking skills. In the last 11 years, she has cut her weight by 82 pounds.

Deisha "Dee" Moore emphasizes the importance of healthy choices, both physically and mentally.

"You look good, you feel good. You feel good, you look good," said Dee.

‘Every time my son came home he was bigger. Now I’m delighted slim James is back’ – Telegraph.co.uk

Posted: August 17, 2020 at 3:59 pm

He changed into something more comfortable for the celebratory dinner, enjoyed the celebrations and the next day said he was going to lose weight.

I was absolutely thrilled but kept that to myself. Instead, I adopted the measured tones my mum used when I started losing weight and simply said: I think you suit being slim and fit much better than being heavy.

Every pound he reported lost I applauded him as did his dad and sisters. He went for healthy portion control and liked the routine of intermittent fasting and as his weight dropped his mood lifted. He changed his university course, stopped beer and takeaways and ended the unhappy relationship.

He embraced exercise again, both aerobic and weight training and we could see the toned, muscular young man starting to reappear. By the time lockdown started with our three youngest back home James was three stone down, and in the intervening period hes lost the last stone.

There was lots of cooking and more drinking than usual going on, but he didnt touch a drop of alcohol for over eightweeks, allowed himself a treat every Sunday and was a total encouragement and support to the rest of us. Many packages arrived with new smaller clothes but no-one begrudged him, especially since his good mood affected all of us.

Im not fattist when I say Im delighted slim, healthy James is back. I no longer worry about all the problems that come with being fat, from poor physical and mental health to bad self image. I love seeing him heading out for a run, or all dressed up for a night out. Weve agreed now to always talk about it, no matter how uncomfortable, if anyone gains weight again.

Body positivity should be encouraged, but its never an excuse for carrying far too much weight. Being fat yes, I use that word - has significant health risks, including premature death, and if vanity doesnt inspire weight-loss, being healthy should.

There are many reasons for people gaining weight, including poverty and being unable to cook, but we need to stop pretending that encouraging weight-loss is fat shaming. The two thirds of the population that are overweight and the 29 per centwho are morbidly obese need help to lose weight and keep it off.I recently interviewed a despairing cardiologist who said fat people risk ending up blind and limbless but no-one would quote him, or even condone his use of the word fat.

Its not fattist to want your children to be fit and healthy rather than fat and miserable, but Im not pretending its easy to lose weight. Im so proud of James, though I wish Id been brave enough to mention it when he was just a stone overweight and save him months of misery.

Missing out on a bucket-list activity pushed this woman to lose 151 pounds – MSN Money

Posted: August 17, 2020 at 3:59 pm

Lindy Cellucci, 60, remembers the exact moment she decided it was time to do something about her weight. It was 2014, and the Toronto native and three of her friends took a girls trip to Canadas east coast.

I was having a hard time keeping up with them, she told TODAY. We were doing a lot of walking and I was finding it harder each day. I had dizzy spells, and I was afraid something might happen to me.

UP NEXT

Their trip included a stop at Hopewell Rocks, a natural formation where you can walk on the ocean floor when the tide goes out, surrounded by sandstone structures that look like flowerpots. One thing I really wanted to do was go to the Rocks. It was on my bucket list, she said.

But it was a hot day, and reaching the ocean floor meant a steep walk down and back up. I just knew I couldnt do it, she said.

Her friends felt terrible, but Cellucci encouraged them to go on without her. I watched them go. I could see them way, way down, running around on the ocean floor, taking selfies, laughing and joking. I was up top with the great-grandmothers. I thought, This is stupid. Youre missing out, she said.

I said out loud, Its time. Im stopping everything right now and Im going to fix this.

And she did. With five days left in her vacation, she stopped eating junk food and started making healthier food choices.

When she got home she stepped on the scale for the first time in years. The scale only went to 300, she said. I had no idea my weight was starting with a 3. It was mortifying.

She joined Weight Watchers (now WW) and followed its healthy eating plan. Right away I started to lose some weight and feel better, she said.

This was the first time in all the weight-loss programs I tried where I realized it was not just a quick fix this was going to be forever. I wasnt starving myself or eating just bananas or just potatoes. I was cooking wonderful food and creating wonderful recipes that were healthy and delicious, she said.

To avoid temptation, she keeps trigger foods out of her house. Im a food addict. That hasnt changed, she said. I can say no once in the store, or 400 times in the house.

Exercise was a challenge. Because I was a very big woman it was hard to start exercising. I didnt do that for about three months, she said. For Christmas, her sister bought her a Fitbit and she started walking one block, which was about 1,000 steps. My back would hurt and my knees would hurt, she said. But she enjoyed competing with other Fitbit users. I liked the feeling of getting ahead, she said.

As the weight came off, I could add an extra lap, and more weight came off, she said. I kept walking, and I started to go to yoga, Zumba, boot camp and swimming. I was feeling so stoked. She worked her way up to 25,000 to 30,000 steps a day and was halfway to her goal weight by the end of 2015.

Cellucci hit a serious stumbling block breast cancer. In December 2015 after a cold walk, she tucked her hands up under her arms to warm them. And she felt a lump. From then, everything happened quickly ultrasound, mammogram, biopsy and surgery. She needed months of chemotherapy and radiation and medication through a port in her chest.

The worst part of having breast cancer was not losing my hair, not feeling physically ill, not being afraid to die. The worst part for me was I put every last pound back on.

The steroids she had to take led to weight gain, and chemotherapy made most foods taste lousy, so she said she lived on fried foods and salt. The worst part of having breast cancer was not losing my hair, not feeling physically ill, not being afraid to die. The worst part for me was I put every last pound back on, she said.

Cellucci still wore her Fitbit, and she rejoined WW around the time of her second all-clear mammogram. Slowly, the pounds came off again. After 20 months of losing pretty much every single week, in September 2019 I reached my goal I lost half of me, she said. I look better at 60 than I did at 40. She now weighs 149 pounds and has gone from a size 22 to a size 9.

She was even featured on the cover of People magazines Half Their Size issue. For a whole week I hung out at the grocery store where the magazine was on the rack telling strangers, Thats me! she said.

And as for her trip to Hopewell Rocks? Shes planning a visit once the pandemic eases. As soon as I can travel, Im going to run up and down those stairs, she said.

Cats need to see the veterinarian regularly, too – Marin Independent Journal

Posted: August 17, 2020 at 3:59 pm

When I was a child my family always had cats. But as I look back, I realize we werent always the best pet guardians. Sure, we loved the many cats we had but beyond getting them spayed or neutered, they never saw a veterinarian again until they were extremely sick. To make matters worse, we let them go outside whenever they wanted, which put them at even more risk for health problems.

A recent study found that 45% of people never take their cat to the veterinarian unless its an emergency. With National Take Your Cat to the Vet Day coming up on Aug. 22, its a good time to be reminded that being a good pet parent means committing to regular, lifelong checkups for our feline buddies.

Like all pets, cats benefit from a yearly checkup with a veterinarian who can provide ongoing wellness care and detect small problems before they become more painful or difficult or expensive to treat, says Belinda Evans, director of shelter medicine at Marin Humane. Also, cats tend to hide discomfort and pain so by the time theyre exhibiting obvious signs, they could be very ill.

Cats are often perceived as independent and tough. Unfortunately, that can give us the impression they dont need a lot of watching over. Combine that with the oftentimes difficult task of getting a cat in a carrier and its easy to see why some dont place a priority on regular visits to the veterinarian.

But our tough cats can fall victim to several serious illnesses. Viruses and bacteria can invade a cats nose, throat and sinuses, causing upper respiratory infections. In multi-cat homes, viruses are passed through shared food or water dishes, grooming or sneezing. Stress can contribute to URIs, signs of which include a runny nose, congestion, nasal discharge, fever, loss of appetite and rapid breathing.

Feline urinary tract disease is characterized by blood in the urine and painful urination. Cats experiencing urinary problems may strain while urinating, squatting and meowing in pain. But with prescribed medication from a veterinarian and a change in diet, these infections should be easy to treat.

An often-neglected part of medical care for our kitties includes dental care. Feline gingivitis, or gum inflammation, is the earliest stage of periodontis. Gingivitis is caused by plaque accumulating along the gumline. Left untreated, tartar eventually builds up under the gum, separating it from the teeth. Reddened gums, bad breath and difficulty eating are early signs of gingivitis, which usually begins with one tooth but can spread quickly. This bacterium can spread to other organs through the bloodstream and cause serious kidney damage.

Other illnesses seen in cats include hyperthyroidism and cardiomyopathy both of which can be fatal if left untreated.

The most common cause of death for cats is chronic kidney (renal) failure. Because it occurs gradually, cats may show no signs for years. By the time they exhibit symptoms, damage is usually irreversible. Extreme thirst and frequent urination are among the signs, along with drooling, weight loss and bad breath. Although incurable, a cat can be kept comfortable with dietary changes, along with IV fluids and specific medications.

Most veterinarians agree that a combination of high-quality dry and wet food is ideal for cats. And keeping them indoors ensures you as their pet parent can monitor their health better (not to mention keeping them safe from cars, coyotes and fights with other cats or dogs).

Our pets give us so much and ask little in return. Lets keep them as healthy and pain-free as wed like all our two-legged family members to be.

Palou is 7th in the Fast 9 of Indy500 after a technical slip, Pole Andretti – Explica

Posted: August 17, 2020 at 3:59 pm

Saturday was the fastest and Sunday he dominated again. Marco Andretti has achieved the Pole Position of the Indianapolis 500 in the Fast 9, achieving the first pole for the surname in 33 years. It has been imposed on Scott Dixon and Takuma Sato. Alex Palou was seventh after experiencing a loss of pace due to a problem with the weight jacker.

The nine fastest cars in the qualifying session on Saturday have been the ones that have won the ticket to the Fast Nine, in which this Sunday the Pole Position has been decided. They have gone out to shoot in reverse order to the record of the previous day. Thus, as Palou had been seventh, today he was the third to take the asphalt.

The person in charge of opening the track has been Takuma Sato, who has flown four consecutive laps in 230 to complete his attempt with an average of 230,725 miles / hour, with hardly any loss of pace. Then it was the turn of Graham Rahal, who has stayed at 229,380 miles / hour and has reported feeling uncomfortable with the car.

Then its time to Palou. The Team Goh rider was spectacular on his first lap 231.901 miles / hour, the best first lap of the day but he quickly lost pace in the following and finished with an average of 229.676 miles / hour. The reason for this decline has been a problem with the weight jacker a sort of weight distribution.

The weight jacker is a control that the pilot has on the steering wheel that affects the rear suspension and the distribution of weights. On the straights, it allows you to lower the suspension and raise the nose, which ultimately reduces downforce and provides a plus of about 0.5 miles / hour. When it returns to its original position, the car gains aerodynamics.

That slip was very expensive for the Team Goh rider, who this Saturday had reached 231.034 miles / hour and who felt he had the potential to approach 231.5. And it allowed him to lose ground to the other rookie of the session, Rinus VeeKay, who has completed his stint at 230,704 miles / hour.

We had a couple of problems with the weight jacker. We have to check if it was me or if it was the system, but its a shame because the car had the speed and the team had done a great job. Now I look at the race. . Im sure it will be very good, he told the American broadcast as he got out of the car.

But Sato would not be the fastest of the session, because Scott Dixon would climb twice to 231 to complete his attempt in 231,051 miles / hour, placing himself in the provisional pole position awaiting the four cars of the Andretti team, who had dominated Saturdays qualifying with a poker.

The first Andretti to go out on the road was James Hinchcliffe, who posted a discreet average of 229,870 miles / hour. Worse luck was Alexander Rossi, finishing last with a 229,234 that suggested the track slowed down as temperatures rose. Ryan Hunter-Reay would go up with 230,648, but still far behind on Saturday.

If the first three Andretti had been hesitant, Marco Andrettis was the opposite. The fastest man on Saturday would climb twice to 231 and complete his attempt with an average of 231,068 miles / hour, enough to pass Dixon by the minimum 0.017 miles / hour and score the Pole Position of the Indianapolis 500 .

Families feel crushing isolation of nursing homes – Brownsville Herald

Posted: August 17, 2020 at 3:59 pm

By Sarah R. Champagne,Texas Tribune

After 70 years of marriage, the coronavirus tore Margie and Werner Stalbaum apart. But Margie, who was positive for COVID-19, wasnt the one who died. It was Werner, of natural causes and maybe of loneliness.

In early June, when 87-year-old Margie tested positive for the virus in the Cedar Park nursing home where they lived together, she was transferred to a different facility in nearby Round Rock to be isolated.

When their granddaughter Serena Bumpus visited Werner during that period, she talked to him through a window. Werner, who was 88 years old with dementia, would point at his wifes empty bed, looking as if he didnt know what was going on.

Part of me wonders, and the rest of my family wonders, did he think she had already passed? said Bumpus, who is a nurse. And he just thought, Its time for me to go be with her.

The coronavirus pandemic has been a constant and precarious balancing act between limiting the spread of the virus and the need for life to go on. In few places has this balance been more delicate than in long-term care facilities, where elderly and medically fragile residents have been deprived of visits from loved ones for almost five months.

For some families, that wait is ending as the state rolls out new rules to allow visitation again in certain nursing homes and assisted living facilities, but it remains unclear how many facilities can or will start allowing visits. And some families say the damage to their loved ones from prolonged isolation has already been done.

As the pandemic reached the U.S. and began ravaging nursing homes soon after most states with coronavirus outbreaks closed visitation at long-term care facilities. Recently, some states have begun allowing visitors again as the COVID-19 curve flattened.

In Texas, Gov. Greg Abbott shut down visitation in mid-March. That order remained in effect for 145 days until Aug. 6, when the state eased restrictions for facilities that dont have any active COVID-19 cases among residents or confirmed cases among staff in the last two weeks.

Of Texas 1,215 nursing homes, 56% still had active cases on Thursday while more than 15% of the 2,000 assisted living facilities have reported active infections.

Once a facility determines it can allow visitors, the next step is to get approval from the state if it decides to resume visitation and thats up to each facility, Texas Health and Human Services spokesperson Kelli Weldon said in an email. Weldon added that the state doesnt yet have a list of facilities that have been approved to resume visitation.

Its up to families to contact the facilities to find out whether they are able to allow limited visitation. Even facilities that meet the requirements cannot allow physical contact between residents and visitors, state officials said.

For many families, this is not acceptable.

My mom has Alzheimers, she is nonverbal, says Genny Lutzel, whose mother Paula is in an assisted living facility in Rockwall, near Dallas. Everything in her world is sensory, sensory touch, sensory communication. And we cant touch.

COVID-19 has been so devastating in long-term care with close to 22,000 infections and over 3,100 deaths in Texas since the beginning of the pandemic that facilities are fearful of allowing any visitors and wary of putting more pressure on their staff, who will have to supervise every minute of the visits, said Jude Goodson, former executive director of Orchard Park at Southfork, an assisted living facility south of Houston.

Goodson said the pandemic has put facilities under tremendous financial pressure because of expenses like protective equipment for staff and technology to keep the residents in touch with their families. Meanwhile, revenues have dropped because of fewer new admissions and more deaths, she said.

With severe financial issues, where is that extra staff [to manage visitations] going to come from? Goodson said.

Roadblocks to visits

Abbotts March order halting visitation didnt halt the spread of COVID-19 among some of the states most at-risk residents. After the governor allowed businesses to gradually reopen in May and June, infections in nursing homes and assisted living facilities soared. In July, more than 11,000 of their residents were infected, and 1,350 died more than four times the totals for June.

While families were banned from entering the facilities, infected staff members brought the virus to work with them, health experts say. Once inside a facility, it spreads like a wildfire, Phil Wilson, the acting executive commissioner for the Texas Health and Human Services Commission, said during a webinar about the new visitation rules on Aug. 7.

The visitation rules are taking effect even as cases in nursing homes and assisted living centers are still growing, with deaths in long-term care facilities still making up for a third of the states overall toll. More than 1,100 new cases have been reported over the past week, a 54% drop compared to the last week of July which saw some of the highest case numbers of the pandemic but still more than double the weekly average for June.

Under the new rules, in addition to being COVID-free, nursing homes the hardest hit facilities will also have to test their staff every week, which has been difficult to achieve because of limited access to testing.

Testing has been an ongoing challenge, Kevin Warren, president and CEO of the Texas Health Care Association, which represents long-term care facilities, said last week after the new rules were published. Facilities can perform their own testing, using federal funds allocated for COVID-19, but without that federal money, Warren said it can cost facilities up to $15,000 a week to perform tests.

In July, the state tested all residents and staff in only about 7% of long-term care facilities, either through requests by the facilities or through quick response teams the state deployed after outbreaks were reported. In August, the state plans to test residents at 9% of nursing homes and assisted living facilities.

At the end of July, the federal Centers for Medicare and Medicaid Services started sending devices to nursing homes that can perform antigen tests on-the-spot within minutes; 372 have been allocated so far at Texas more than 1,200 nursing homes. Antigen tests are taken by nasal or throat swab like other tests, and while faster, they generate more false negative results than other kind of test.

But the new requirement to test all staff weekly could be a Catch-22 for nursing homes: those with no active case arent prioritized to receive those testing devices.

I dont see how a nursing facility can test staff weekly without point-of-care testing [with the federally-supplied devices], said Patty Ducayet, the states long-term care ombudsman, adding that she has no evidence that the state is fulfilling the remaining need.

Isolation kills too

Lutzel said she has been visiting her mom through a window since March.

I know theyre doing everything they can, but there is just no substitute for a family member, she said.

Scientists have long studied the effect of social interaction on the brain; the pandemic has offered a grim occasion to measure the consequences of the lack of interaction.

Isolation can lead to mental and physical decline, said Dr. Carmel Dyer, professor and executive director of the Consortium on Aging at UTHealth in Houston. Anxiety and depression increase with social isolation.

One thing that our brains like the most is social interaction, says Dr. Janice Knebl, professor in geriatrics at the University of North Texas Health Science Center at Fort Worth.

Both said several of their patients in long-term care have shown signs of declining health at a much faster pace than the normal course of aging or dementia.

Leora and Aretha Carter have also noticed the rapid decline of their mom, Willie Mae Carter, who is in Ridgecrest Retirement and Healthcare in Waco, during their weekly through-the-window visits.

She had dementia but could still recognize us even if it took a minute. Now she wont even get up out of a chair. I understand shes 90 years old, but it occurred so quickly, Leora Carter said.

She had a chance to do an outside visit with her mother in mid-May, but being six feet apart was hard, and the ritual hug to say goodbye was impossible.

Dyer said people with moderate to severe dementia are not always aware of the reasons why their loved ones cant visit or hug them and might feel abandoned.

The new state rules allow for a failure to thrive exception to the visitation ban, which has to be documented and is only allowed at facilities that meet the other requirements.

Under the exception, if a physician diagnoses a decline in a residents physical or mental health, one person can be designated to be the sole visitor for that person, and not just in end-of-life situations as has been the case.

Signs of a failure to thrive include weight loss, decreased appetite, poor nutrition, and inactivity, reads the new emergency rules for nursing facilities.

For Renee Griggs, the fact that rules are different for different facilities creates a lot of confusion.

Her mother suffers from dementia and lives in an assisted living facility called The Grandview of Chisholm Trail in Fort Worth. When she talked to the facility on Friday, they didnt know if they could or were going to allow visits.

Griggs said her mother has lost 16 pounds since January, and shes gone from remembering her daughter to being disoriented and incontinent.

Even though COVID itself is not killing my mom, the consequences of the disease are killing her, she said.

She picked her mother up last week on Friday for an essential doctors appointment. She still has not driven her back to the facility. I just couldnt do that, she said.

The Stalbaum family thinks Werner Stalbaum could be another victim of the virus who never contracted it.

Margie Stalbaum was her husbands lifeline, and putting her in quarantine took away the one person who kept things familiar, Bumpus said.

They had never been apart for so long, she added, and Werners health declined quickly after his wife was transferred.

Bumpus had a chance to visit her grandfather in his last hours, because of the exception to the visitation ban for compassionate end-of-life care. Bumpus said what she saw still haunts her; in 18 years of nursing, she said she had never seen such a look of defeat on everyones face, including residents and the staff.

That day, Margie Stalbaum was still waiting for a second test to come back negative so she could be reunited with her husband. She learned through a wrought iron fence, through masks and distance, that the love of her life was gone.

She could hardly formulate a sentence after she returned to the Cedar Park facility, Bumpus said, adding that she believes isolation caused her grandmother to become disoriented.

Only at Werners funeral did it become clear that Margie didnt fully grasp what had happened.

And so when she is rolled up to the casket to say goodbye, she looks at my aunt and says Oh my god, he died, Bumpus said.
