It was a hot summer evening in 2008 when I received a panic call from a friend. She was breaking down and couldnt explain herself clearly. All I could figure out was that her 24-year-old son Saurabh had been found to have a blood sugar of 400+. Could he really be a diabetic? What would be his future? I saw Saurabh the same evening. He did not seem sick at all, except that he was about 20 kg overweight, despite losing some weight over the last few weeks.

Saurabhs HbA1c concentration was 13 per cent. HbA1c is a measure of blood sugar as a three-month average. The normal range is <5.7, 5.7-6.4 is considered prediabetes, and 6.5 or above is labelled as diabetes. As is the protocol for young patients with such high blood sugar values, insulin treatment was initiated. His laboratory tests came back and he was found to have Type 2 diabetes, the common type of diabetes that usually affects older adults and does not always require insulin for treatment.

Saurabh worked in a law firm, had 10-14 hour days, ate in the office, partied on weekends. Sleep hours were erratic and interrupted. He had a family history his mother had developed diabetes at the age of 50. Diabetes hit him earlier because of his obesity and lifestyle.

Since that day, Saurabh turned a new leaf. He gave up junk food altogether and started regular exercises. He lost 12 kg over the next six months with lifestyle modification and behavioural discipline. In a few weeks, his insulin was discontinued. His blood sugar values remained under control on progressively lower doses of oral medication. Another six months passed and Saurabh lost an additional 6 kg. Soon, he was able to maintain normal blood sugar levels without any medication. To date, he continues to monitor his blood sugar and all his other parameters every few months. All parameters are within range. He has not regained his body weight. For the last 12 years he is off all medication and continues to be normal.

Is this a miracle? Can this be reproduced? Let me first state that some patients of Type 2 diabetes can go off medication, and remain fine for years. Its not called cure or reversal, which implies permanency. Rather, the term remission is used, since we dont know if it is permanent. Type 2 diabetes is an ongoing, progressive disease and it is expected to make a comeback at some point. Clearly, however, several years of postponement of diabetes is possible. This has tremendous benefits for our long-term health and well-being.

The key to achieving a remission in Type 2 diabetes is weight loss. Weight and diabetes have a direct and powerful connection. Much of the increase in diabetes prevalence worldwide is linked to increasing obesity. Shedding off those extra kilos can reverse many metabolic processes and can induce remission in diabetes, particularly if diabetes is of recent onset. Type 2 diabetes results from a combination of insulin resistance (resistance to insulin action at the cellular level) and insulin deficiency (the pancreatic beta cells are unable to make enough insulin). Weight loss can partially reverse both of these processes.

Studies have shown that very low-calorie diets can help achieve weight loss and remission of diabetes. About 600-800 calories per day, consumed for two to three months, followed by a maintenance diet were able to normalise blood glucose levels in about half the participants and also maintain them, for at least a year.

A large community-based clinical trial called DiRECT (Diabetes Remission Clinical Trial) was conducted in the UK. DiRECT enrolled people with Type 2 diabetes for less than six years and not receiving insulin treatment. They were provided either standard diabetes care or low-calorie meal replacement diet (825850 kcal/day) for three to five months, followed by stepwise food re-introduction and a long-term weight maintenance programme. At a one-year follow-up, 46 per cent of patients had achieved diabetes remission (HbA1c <6.5 per cent without medication). At two years, the remission rate was 36 per cent.

The diets used in these trials were quite restrictive, requiring strong, sustained motivation, and could only be followed under close medical supervision. The long-term effects of such severe diet restriction are unknown at present. Some individuals achieve calorie reduction by following different types of intermittent fasting protocols. Although remission is reported with long standing diabetes too, the shorter the duration of diabetes (usually less than 5-6 years) the greater the chances of success.

What about exercise? Exercise keeps us healthy in many ways and helps control our blood glucose too. But exercise alone, even 10,000 steps a day, is not enough to induce remission. It has to be combined with calorie restriction.

Weight loss (bariatric) surgery has been known to induce remission in patients with diabetes. The signs of diabetes reduce or disappear within a few days, even before any major weight loss, suggesting that severe limitation in food intake or changes in gut hormones could be a reason. In the long term, weight loss seems to be the main factor responsible for better diabetes control.

Are such aggressive measures always required to induce remission in diabetes? In our experience, many patients like Saurabh who are overweight or obese and initiate calorie restriction (not necessarily drastic, as in the UK trials), early in the course of diabetes, are able to achieve remission. I dont believe it is always necessary to resort to very low-calorie diets. Any combination of diet and exercise that helps one knock off the extra kilos in a young individual with recent onset diabetes has the potential to induce remission. Even a 10 per cent patient weight loss is sufficient in some cases to normalise blood glucose.

More recent approaches that have shown good results include custom-designed diets and programmes based on a large number of data points acquired through continuous monitoring of body parameters.

Diet can influence mood, behavior and more – Japan Today

During the long seafaring voyages of the 15th and 16th centuries, a period known as the Age of Discovery, sailors reported experiencing visions of sublime foods and verdant fields. The discovery that these were nothing more than hallucinations after months at sea was agonizing. Some sailors wept in longing; others threw themselves overboard.

The cure for these harrowing mirages turned out to be not a concoction of complex chemicals, as once suspected, but rather the simple antidote of lemon juice. These sailors suffered from scurvy, a disease caused by a deficiency of vitamin C, an essential micronutrient that people acquire from eating fruits and vegetables.

Vitamin C is important for the production and release of neurotransmitters, the chemical messengers of the brain. In its absence, brain cells do not communicate effectively with one another, which can lead to hallucinations.

As this famous example of early explorers illustrates, there is an intimate connection between food and the brain, one that researchers like me are working to unravel. As a scientist who studies the neuroscience of nutrition at the University of Michigan, I am primarily interested in how components of food and their breakdown products can alter the genetic instructions that control our physiology.

Beyond that, my research is also focused on understanding how food can influence our thoughts, moods and behaviors. While we cant yet prevent or treat brain conditions with diet, researchers like me are learning a great deal about the role that nutrition plays in the everyday brain processes that make us who we are.

Perhaps not surprisingly, a delicate balance of nutrients is key for brain health: Deficiencies or excesses in vitamins, sugars, fats and amino acids can influence brain and behavior in either negative or positive ways.

Vitamins and mineral deficiencies

As with vitamin C, deficits in other vitamins and minerals can also precipitate nutritional diseases that adversely impact the brain in humans. For example, low dietary levels of vitamin B3/niacin typically found in meat and fish cause pellagra, a disease in which people develop dementia.

Niacin is essential to turn food into energy and building blocks, protect the genetic blueprint from environmental damage and control how much of certain gene products are made. In the absence of these critical processes, brain cells, also known as neurons, malfunction and die prematurely, leading to dementia.

In animal models, decreasing or blocking the production of niacin in the brain promotes neuronal damage and cell death. Conversely, enhancing niacin levels has been shown to mitigate the effects of neurodegenerative diseases such as Alzheimers, Huntingtons and Parkinsons. Observational studies in humans suggest that sufficient levels of niacin may protect against these diseases, but the results are still inconclusive.

Interestingly, niacin deficiency caused by consumption of excessive amounts of alcohol can lead to similar effects as those found with pellagra.

Another example of how a nutrient deficiency affects brain function can be found in the element iodine, which, like niacin, must be acquired from ones diet. Iodine, which is present in seafood and seaweed, is an essential building block for thyroid hormones signaling molecules that are important for many aspects of human biology, including development, metabolism, appetite and sleep. Low iodine levels prevent the production of adequate amounts of thyroid hormones, impairing these essential physiological processes.

Iodine is particularly important to the developing human brain; before table salt was supplemented with this mineral in the 1920s, iodine deficiency was a major cause of cognitive disability worldwide. The introduction of iodized salt is thought to have contributed to the gradual rise in IQ scores in the past century.

Ketogenic diet for epilepsy

Not all dietary deficiencies are detrimental to the brain. In fact, studies show that people with drug-resistant epilepsy a condition in which brain cells fire uncontrollably can reduce the number of seizures by adopting an ultralow-carbohydrate regimen, known as a ketogenic diet, in which 80% to 90% of calories are obtained from fat.

Carbohydrates are the preferred energy source for the body. When they are not available either because of fasting or because of a ketogenic diet cells obtain fuel by breaking down fats into compounds called ketones. Utilization of ketones for energy leads to profound shifts in metabolism and physiology, including the levels of hormones circulating in the body, the amount of neurotransmitters produced by the brain and the types of bacteria living in the gut.

Researchers think that these diet-dependent changes, especially the higher production of brain chemicals that can quiet down neurons and decrease levels of inflammatory molecules, may play a role in the ketogenic diets ability to lower the number of seizures. These changes may also explain the benefits of a ketogenic state either through diet or fasting on cognitive function and mood.

Sugar, saturated fats and ultraprocessed foods

Excess levels of some nutrients can also have detrimental effects on the brain. In humans and animal models, elevated consumption of refined sugars and saturated fats a combination commonly found in ultraprocessed foods promotes eating by desensitizing the brain to the hormonal signals known to regulate satiety.

Interestingly, a diet high in these foods also desensitizes the taste system, making animals and humans perceive food as less sweet. These sensory alterations may affect food choice as well as the reward we get from food. For example, research shows that peoples responses to ice cream in brain areas important for taste and reward are dulled when they eat it every day for two weeks. Some researchers think this decrease in food reward signals may enhance cravings for even more fatty and sugary foods, similar to the way smokers crave cigarettes.

High-fat and processed-food diets are also associated with lower cognitive function and memory in humans and animal models as well as a higher incidence of neurodegenerative diseases. However, researchers still dont know if these effects are due to these foods or to the weight gain and insulin resistance that develop with long-term consumption of these diets.

Time scales

This brings us to a critical aspect of the effect of diet on the brain: time. Some foods can influence brain function and behavior acutely such as over hours or days while others take weeks, months or even years to have an effect. For instance, eating a slice of cake rapidly shifts the fat-burning, ketogenic metabolism of an individual with drug-resistant epilepsy into a carbohydrate-burning metabolism, increasing the risk of seizures. In contrast, it takes weeks of sugar consumption for taste and the brains reward pathways to change, and months of vitamin C deficiency to develop scurvy. Finally, when it comes to diseases like Alzheimers and Parkinsons, risk is influenced by years of dietary exposures in combination with other genetic or lifestyle factors such as smoking.

In the end, the relationship between food and the brain is a bit like the delicate Goldilocks: We need not too little, not too much but just enough of each nutrient.

Wheels didnt share what he weighs now, but compared to posts from earlier in the year, hes far more trim, with more noticeable veins and striations and less bulk. One of the biggest differences is in his face, which looks a good deal thinner than photos from just a few weeks ago.

In an Instagram post from August 16, 2022, Wheels wrote about going from 300 pounds down to 240. That was before his steroid and TRT announcements, so its possible hes even lighter now.

Wheels mentioned recently that he is pumping the brakes on his journey to deadlift 1,000 pounds which is in part due to his decision to stop taking steroids. A recent back injury played a part in it, too. In the meantime, he recently posted a video of himself and bodybuilder Andrew Jacked working out shoulders and bicepsin Dubai.

In the video, Wheels mentions that his skin has cleared up now that hes off a steroid cycle and that hes expecting to start dropping more water weight soon after. He also says that his strength in the gym hasnt been impacted yet, but hes expecting that to come in the next few weeks.

With Wheels being such an open book on social media, expect more updates on his post-steroid life in the months to come.

The majority of expertsincluding the Centers for Disease Control and Preventionunanimously agree that an average of one to two pounds of weight per week is a healthy weight loss goal. In fact most of them, including Jenna Kilgore, MS, NASM CPT, FNS andNoom Coach, maintain that suddenly losing a lot of weight is less sustainable than slow and steady weight loss.

Dr. Wajahat Mehal, MD, PhD, weight loss expert with Yale Medicine and director of the Weight Loss Program and professor at Yale School of Medicine, suggests looking at the big picture, focusing on how much weight you can lose overall versus how much weight you can lose in a week. Total weight loss with lifestyle intervention is generally 3-4% of starting weight, he explains.

There are a number of factors that contribute to weight loss, which may result in a more rapid or gradual weight loss. Weight loss is a complicated process, kind of like putting a puzzle together, adds Kilgore. In other words, you may be able to lose more or less than the average one to two pounds per week due to these variables. Here's everything you need to know about how to do it safely.

7 variables that contribute to weight loss

1. Body mass

Kilgore explains that someone who is starting off at a higher BMI is more likely to lose more weight in a week than someone with a lower one. Additionally, while science has concluded that muscle doesnt actually weigh more than fat (a pound of muscle weighs the same as a pound of fat) it is denser. This is why most health and fitness experts rely more on body composition measurements than simply the weight on the scale.

2. Water weight

Water weight is more of a sign of quick weight loss than sustained weight loss, explains Gans. Since our body is made up of 60% water it is not uncommon to see weight fluctuations from one to five pounds in a given day. Bottom line: You shouldnt consider water weight loss true weight loss, because it will come back as soon as you hydrate.

When attempting to lose weight, most people focus on calorie intake. While seriously restricting caloric intake will inevitably lead to rapid weight loss, experts suggest taking a more moderate and sustainable approach with your total number of calories consumed per day dependent on how many you burn. For example, a more active person who is regularly exercising needs more calories than someone who is sedentary. However, overall a 500 calorie deficit per day should lead to one pound per week of weight loss, maintains Gans.

A doctor or weight loss expert can help you determine a healthy deficit goal using bioimpedance analysis, or BIA, which uses factors such as your muscle mass and basal metabolic weight to figure out what your caloric deficit should be.

4. Nutrition

When it comes to sustainable weight loss and food, calories arent the only thing that matters. Nutrition is the most important aspect of a healthy weight, stresses Dr. Mehal. Without changing the home food environment it is very difficult to have significant weight loss.

In the Dietary Guidelines for Americans 20202025, the USDA explains that a healthy eating plan is not only one that stays within your daily calorie need but involves fruits, vegetables, whole grains, and fat-free or low-fat milk and milk products, protein-rich food including seafood, lean meats and poultry, eggs, legumes (beans and peas), soy products, nuts, and seeds, and is low in saturated fats, trans fats, cholesterol, salt (sodium), and added sugars.

5. Movement

Movement is essential in any wellness journey. Finding ways to move throughout the day, whether that be short bursts or longer, more intentional workouts, or both, can have a lot of benefits, says Kilgore. Exercise is very beneficial for health even without weight loss, reminds Dr. Mehal. Not only does it help burn calories, but promotes mental and physical health, releasing feel-good hormones. 6. Sleep

While stress is normal, too much stress can wreak havoc, specifically on our hormones. A prolonged increase in cortisol is linked to abdominal fat storage, Kilgore explains. Not only can stress affect our hunger and fullness cues which can lead to overeating, it can also result in reaching for those comfort foods and ignoring portion sizes. Chronic stress often shows up as a plateau in progress, she says.

Fill up on fiber

Fibrous foods are your friend when it comes to sustainable weight loss, says Gans. Fiber-rich carbs, such as 100% whole grains, fruits and veggies, take longer to digest in the body than refined carbs causing more satiety at mealtimes, she says.

Dont eliminate foods you love

Instead of ditching your favorite foods, Gans suggests modifying them. For example, dont stop eating pasta, and instead try to make it healthier. Swap your go-to huge bowl of fettuccine Alfredo, for a healthier dish with one-cup cooked pasta tossed with sauted spinach and grilled shrimp. Or, instead of eating a whole pizza topped with sausage and pepperoni, limit yourself to two slices with spinach and mushrooms, supplementing it with a large mixed green salad on the side. Want French fries with the burger? Go for it, but perhaps lose the bun, she advises.

Focus on progress, not perfection

Trying to eat healthy 100% of the time is unrealistic and unwarranted, says Gans. Instead, focus on a more realistic and smarter approach, such as making healthy choices 85% of the time, and allowing yourself to indulge in whatever you wish the remaining 15%. Dr. Mehal agrees that mindset is key. Most people have too high expectations and then feel as though they have failed, he states.

Try the plate method

Gans also suggests creating most of your meals with the plate method in mind, filling your dish up with one-half veggies, one-quarter carbohydrate, and the remaining one-quarter protein and a fat serving. By default, your portions will be more in check and the protein and fat will help fill you up, she says.

Stay hydrated

The more water you drink, the less you will eat, maintains Gans. Many times individuals confuse thirst for hunger. Simply try and stay hydrated by drinking more water and/or unsweetened, low-calorie beverages, she suggests.

Focus on the whys of weight loss

Kilgore stresses the importance of staying connected to the whys instead of the hows. Remembering why losing weight or reaching our goals is important can go a long way, she says. Motivation can be fleeting and that is one of the biggest challenges we see. When a motivational low hits, it can be hard to power forward. Learning to navigate those ebbs and flows will be valuable in the long run.

Find a support system

Its so important to have a strong support system to reach your weight loss goals. The process can feel lonely at times, Kilgore explains. She suggests building up a support system of family members, friends, coaches, and anyone who can help provide motivation and accountability.

Bottom line: Weight loss is an individualized journey

Remember that there is not a one-size-fits-all approach to weight loss and how much weight you can lose in a week. Everyones journey to weight loss is their own, notes Kilgore, who recommends keeping the focus on sustainability over speed. Death of Congressman’s Wife’s Prompts Concerns Over White Mulberry Leaf Safety – Everyday Health

Autopsy results for Lori McClintock, who died last year, are raising questions about herbal supplements after findings implicated white mulberry leaf in her death.Lori McClintock was the wife of U.S. Rep. Tom McClintock.

According to the March 10 report from the Sacramento County coroner, which was recently obtained by Kaiser Health News (KNH)and released this week, the cause of death was gastroenteritiscaused by adverse effects of white mulberry leaf ingestion.

The report noted that a partially intact white mulberry leaf was found in McClintocks stomach, but it wasnt clear from the autopsy report whether she drank a tea with white mulberry leaves, ate fresh or dried leaves, or took a dietary supplement containing white mulberry leaf, KHN reported. The death was ruled accidental in the autopsy, according to KHN.

Several toxicologists and doctors are skeptical that white mulberry leaf would cause death or even serious illness, however.

After reading the actual report which clearly states that its the cause I still don't believe that's an accurate description of what happened, says Pieter Cohen, MD, an associate professor at Harvard Medical School in Boston, who leads the dietary supplement research program at Cambridge Health Alliance. If you eat any leaves, theyll upset your stomach, but they wont kill you, Dr. Cohen says.

White mulberry is a tree that is native to China, Korea, and Japan, but is now found widely across Asia, Europe, and the Americas. Its been used for centuries as a traditional Chinese herbal remedy for a wide range of ailments, including diabetes, heart disease, dementia, and obesity, says Kelly Johnson-Arbor, MD, co-medical director of the National Capital Poison Center in Washington, DC.

White mulberry leaves contain many biologically active compounds, including alkaloids, flavonoids, and antioxidants that may have beneficial health effects, Dr. Johnson-Arbor says.

When consumed, these compounds may help prevent cell damage, improve glucose levels, and enhance digestion, Johnson-Arbor says. White mulberry tree leaves are also rich in protein and fiber, and have been used as a food source for both people and animals, she adds. In Korea, powdered white mulberry leaves are used to make ice cream!

Decades of research have identified a wide range of potential health benefits associated with ingestion of white mulberry leaf, according to a review of studies to date that was published in 2021. Many of these studies, however, have been done in animals or contained too few people to draw broad conclusions about whether ingesting white mulberry leaf directly improves health, this review shows.

White mulberry leaf has been studied for high cholesterol, obesity, impaired glucose tolerance, and diabetes; however, the evidence is generally insufficient, says Grant Chu, MD, an assistant clinical professor of medicine at the David Geffen School of Medicine at the University of California in Los Angeles, and a medical reviewer for Everyday Health.

The best available evidence on white mulberry leaf has been its role in diabetes, Dr. Chu says. Compounds in white mulberry leaf may help slow the digestion of carbohydrates in the small intestines, Chu notes. This can help to manage blood sugar levels.

In one small clinical trial, scientists gave white mulberry leaf extract to 37 healthy adults and found it reduced blood sugar spikes that typically occur after consuming carbohydrates. Another clinical trial found white mulberry leaf extract helped control blood sugar in 12 adults with slightly elevated blood sugar levels.

And a separate clinical trial of 46 overweight adults found a low-calorie diet combined with white mulberry extract helped people shed significantly more weight than dieting on its own. People given mulberry extract lost about 10 percent of their body weight over three months, compared with about 3 percent for those in the control group that didnt receive this extract.

Of course, these are all extremely small trials, and the evidence is insufficient to formally recommend white mulberry leaf for this purpose without additional rigorous research.

Over the past decade, there have been no deaths associated with the white mulberry plant reported in the United States, says Kaitlyn Brown, PharmD, clinical managing director of the American Association of Poison Control Centers in Arlington, Virginia.

Between 2011 and 2021, there were only nine reported cases of people seeking medical attention after ingesting white mulberry, Dr. Brown says. Seven of these patients had only minor symptoms and the other two had symptoms that were determined to be unrelated to the plant.

Most cases involved accidental ingestion by children, and the majority of kids experienced no symptoms or only minimal issues that didnt require medical attention, Brown adds.

Deaths or serious illnesses are very unlikely to occur from white mulberry ingestion, Johnson-Arbor says. Mulberry and its extracts are minimally toxic, and symptoms that most often occur after ingestion are mild cases of nausea or abdominal discomfort, she adds.

Lori McClintocks death marks the first on record attributed to white mulberry, several toxicology experts said. To my knowledge, there have been no prior reports of serious side effects or death from white mulberry leaf ingestion in the literature until now, Chu says.

McClintocks case leaves a lot of unanswered questions, says Cohen, who reviewed the autopsy report. Normally, people who use white mulberry leaf as an herbal remedy take an extract they dont eat the leaves, Cohen says. Yet the autopsy claims there was a partial white mulberry leaf in her stomach. I have no idea how they purportedly identified that, Cohen says. To have the actual leaf in ones stomach something odd is going on either the person is eating leaves or a supplement was made with whole leaves rather than an extract.

Not only is that odd, but I also find it very improbable that white mulberry leaves by themselves would lead to a death from dehydration but that is what the coroners report claims, Cohen says.

Its possible that the mulberry leaf was ingested in a supplement that contained another substance that was the real cause of illness in this case, says Christopher Hoyte, MD, medical director of the Rocky Mountain Poison Center in Denver and an associate professor in emergency medicine at the University of Colorado. And its also possible that Lori McClintock had an undiagnosed medical condition that contributed to her death, Dr. Hoyte adds.

Deaths that are directly attributable to mulberry leaf are so rare that it raises questions as to whether this substance is actually the culprit in this case, Hoyte says.

The safest way to take herbal remedies like white mulberry leaf is to find a naturopath who works with a very high quality distributor of plants to properly make the extract, Cohen says. He advises steering clear of commercially produced supplements because what they say on the label wont necessarily match whats really in the product.

White mulberry extracts and similar products that are sold as dietary supplements are not regulated by the U.S. Food and Drug Administration (FDA) for safety, purity, or quality, so the products may be contaminated with other chemicals that may be potentially poisonous, Johnson-Arbor says.

Any supplements you buy should have a USP seal on the package, indicating theyve been independently tested to verify the ingredients, potency, and lack of contaminants, Johnson-Arbor advises.

With white mulberry leaf, any symptoms you experience that go beyond a little mild nausea or abdominal discomfort are a sign you may have consumed something else and that its time to contact poison control for help, Johnson-Arbor says.
